How to treat different types of anal fistula

  We have removed the important contraindicated links affecting the surgical treatment of anal fistula and identified the main causes of the formation of anal fistula, making it less complicated and less difficult to perform. The postoperative follow-up rate is more than 95%, regardless of whether the fistula is high or low: the one-time surgical cure rate is more than 95%.  Because of the complex anal fistula, especially the high complex anal fistula, the fistula and the lumen are variable and the adjacent relationship is complex. In the first 100 years of less developed surgical knowledge and technology, surgical treatment of horseshoe fistula or high anal fistula was difficult and faced serious consequences on anorectal morphology and function, which probably did not cure the fistula, but caused fecal incontinence, anal stenosis, long-term failure to heal, gas and fluid leakage, making patients more painful and doctors upset and headache.  In recent years, with the transformation and improvement of surgical techniques and concepts, the diagnosis of anal fistula, complex anal fistula, recurrent anal fistula, refractory anal fistula and high anal fistula has become clearer through medical imaging (mainly high-resolution film magnetic resonance imaging), endoscopic techniques (electronic anal fistula mirror and enteroscopy), medical testing and pathological examination specialization and improvement, providing precise targets and conditions for treatment.  Through clinical experience and summary, I was the first to introduce the term “cystogenic anal fistula” three years ago in the world of anorectology, which provides patients with refractory anal fistulas with an easy understanding of the pathogenesis of the condition and gives anorectal surgeons a direction to study and design treatment methods and prognosis.  To prevent complications, the surgical approach has been improved to eliminate critical contraindications, and the surgical route is clear, easy to follow, and the risk is reduced to near zero. A new level of treatment has been opened up for anal fistula treatment.  The continuous improvement of the analgesic program and the six major concerns plus psychological interventions to dispel pain have long been the shining point of our department, as well as the starting point of a warm and welcoming ward for anal fistula surgery treatment.
