How to assess ovarian function in women

  The ovaries have two major functions in a woman’s life: the production of sex hormones, which maintain a woman’s physical appearance and sexual characteristics, and ovulation. When you go to the hospital for infertility, your doctor will first assess your ovarian function so that you can have a better understanding and grasp of the golden period of fertility and make a good schedule for your fertility.  Age is one of the most important factors in assessing a woman’s fertility. After the age of 35, ovarian function begins to decline, and the older a woman is, the lower her ovarian reserve becomes. Although women’s fertility gradually declines with age, each person’s situation is different, especially for women with premature ovarian failure, they are younger, but their ovarian function is not as good as women in their 40s, so Age alone cannot be used to accurately estimate ovarian function.  Ultrasound can observe the size of the uterus and ovaries, if the uterus is normal and there are some small sinus follicles on the ovaries, it indicates that the ovaries are still functional, if there are only 2 or less small follicles left on the ovaries, or even none, it probably indicates that the ovaries are functional. If there are less than 2 small follicles or even none on the ovaries, it is likely that the ovarian function has significantly decreased. When the uterus is found to be shrunken and the ovaries are so small that they can hardly be found under ultrasound, it is very likely that the ovaries are failing.  Sex hormone test on the 3rd day of menstruation The 2nd-3rd day of menstruation is the most basic and stable time for sex hormones. The reason for choosing to draw blood during menstruation is the same as ultrasound, both are to reflect the basic state of the ovaries.  A higher FSH value on day 3 of the menstrual cycle is indicative of poor ovarian function. If the FSH value on day 3 of the menstrual cycle is greater than 20 IU/L, the chance of pregnancy decreases significantly, and if it exceeds 25 IU/L there is almost no possibility of pregnancy. At this time, the patient may have combined symptoms such as hot flashes, palpitations, insomnia and leg cramps, which require medication and should not be treated with pregnancy assistance. If the FSH value on the 3rd day of menstrual cycle is between 8-10, it shows signs of ovarian function decline and should be treated actively.  IV. Previous IVF ovulation results If you have already undergone IVF treatment but failed to obtain good results with less than 3 or 5 eggs retrieved, it may indicate that the ovaries are not responding well to the medication. More than these three points, the doctor will look at the results of the previous treatment to assess the ovarian potential and will formulate a more appropriate and effective plan for the next cycle accordingly.
