What is nasal reconstruction? What is the need for nasal reconstruction?

The face is the exposed part of the human body. The nose is located in the most central and prominent position of the face, and a slight abnormality, “open to the public”, will give a bad impression, and it cannot be disguised by make-up and other methods. Therefore, once the nose has a defect, it will cause serious heart and great mental pain, and therefore urgently requires treatment. The repair of nasal defects is a kind of surgical modeling art, which must apply the principles and methods of plastic surgery, elaborate design and delicate operation to obtain satisfactory results. The plastic surgeon has to develop a corresponding surgical plan according to the site, scope and severity of the nasal defect, and use different methods to repair it. Among them are flap method, skin grafting method, auricular composite tissue transplantation method, etc. What is the need for nasal reconstruction? The following are the indications for nasal reconstruction: 1, partial or total defect caused by trauma to the nose (including human and animal bites); 2, nasal defect or nasal scar caused by burns (including open fire burns, hot liquid burns and chemical burns); 3, partial or total defect of the nose caused by various congenital malformations, severe saddle nose; 4, partial or total defect caused by infection of the nose; 5, nasal swelling ( including large nevus, hemangioma, etc.) need to be removed surgically or the swelling has been removed causing nasal defect. What are the common methods of nasal reconstruction? The plastic surgeon has to develop a corresponding surgical plan according to the location, scope and severity of the nasal defect, and use different methods to repair it. Among them are flap method, skin grafting method, auricular composite tissue transplantation method, etc. Most nasal defects are clinically severe, and scholars recognize that the frontal flap (also called Indian flap) is the best choice for partial or total nasal reconstruction (because its skin texture and color are consistent with the surrounding area, and the blood supply is good). As the amount of tissue in the frontal flap is often not enough, it will leave a large scar and the color of the flap area is not consistent with the surrounding area, in recent decades, with the wide application of skin tissue expander in clinical practice, our center applies the expander to fully expand the frontal skin first, and then use the expanded frontal flap to repair the nasal defect in the second stage, so that not only the nasal defect is well repaired, but also the skin of the frontal flap area In this way, not only the nasal defect is well repaired, but also the skin of the frontal donor area can be easily and directly sutured without skin implants. In recent years, we have treated many patients with this method and achieved more satisfactory results than traditional methods. Sometimes patients with poor frontal skin or vascular conditions can be treated with microsurgical methods such as upper arm flap or abdominal flap transfer in stages or forearm flap or dorsal foot flap. The choice of surgical approach depends on the patient’s own specific situation. What are the advantages of nose reconstruction with expanded frontal flap method? 1.The texture and color of the skin of the reconstructed nose are consistent with the surrounding area, and the shape is realistic; 2.The scar left on the forehead after the operation is small and not easy to see, and it is important to avoid the more obvious scar on the forehead after the direct cutting of the forehead flap, the inconsistent color of the skin implantation area and the sunken deformity due to the inconsistent thickness; 3.It is also applicable to the patients who are born with low hairline and narrow forehead; 4.The flap tip is long, and there is little tension when it is rotated downward. 4, the flap has a long tip and is not under much tension when rotated downward, which can fully ensure the blood supply of the flap and good survival; 5, at the same time, the tissue at the tip can be used to increase the bridge of the nose, which is especially suitable for oriental people. What kind of examination will the surgeon do for me before surgery? The plastic surgeon will examine the site, extent and severity of your nasal defect, as well as the rest of your body (e.g. forehead, auricle, upper arm, etc.), and take pictures for medical history. You should prepare your medical history, including all medical information, history of past illnesses, drug allergies, and previous treatments, previous nasal surgeries, current medications, etc. It is important that you provide comprehensive and truthful information. How is nasal reconstruction performed with the expanded frontal flap method? The following steps are performed: 1.First of all, frontal expander implantation is performed, which usually requires hospitalization; 2.After the incision is well healed, frontal expander injection is performed regularly, which does not require hospitalization, but usually requires 1-2 times a week, which usually takes 3 months; 3.After the expander is satisfactorily expanded, frontal flap expansion with tip transfer nasal reconstructive surgery is performed in hospital; 4.After about 1-2 months, reconstructive nasal dissection is performed; 5.After 6 months to a year, depending on the situation, nasal reconstructive surgery is performed. After six months to one year, one or more nose reconstruction can be performed until the nose is realistic and the result is satisfactory.
