Talking about ectopic testicles (cryptorchidism)

When every boy is in his mother’s tummy, there are two little balls growing underneath the kidneys (that is, in the baby’s own tummy). As the baby develops into a human, the two balls need to trek through the mountains and follow the path guided by the “fairy” to open up the belly and all layers of muscle tissue to the bottom, like a worker digging a tunnel, against the belly, hard work to dig a tunnel and make a nest for themselves (the tunnel dug out is called “inguinal canal” and the nest made is called “scrotum”). The two eggs, only in their own nest, can enjoy the warm subtropical climate and produce the royal family (that is, sperm) properly.      Some eggs are born with the sign of Sagittarius, like to play, do not like to work, do not follow the guidance of the “immortals”, did not dig a tunnel, like to stay in the belly; some eggs are born like halfway, digging a tunnel, only half dug, stay in the tunnel (that is, stay in the groin tube); worse, run to the neighbor’s house across the street Drinking and playing cards (ectopic testicles). These types of balls are all naughty and mischievous and can cause a lot of trouble for the baby (as shown below).      What will be the problem if the egg does not stay where he should be? 1, the egg’s own nest (scrotum), the temperature is lower than in the belly (the temperature in the center of the human abdominal cavity is definitely higher than the scrotum), he can live a leisurely life and be able to produce tadpoles (sperm) normally. In other words, the belly is a flaming mountain, not only will the eggs produce a lower quantity of sperm, the quality will also decline (this will lead to infertility in adult men). 2, eggs that do not stay in their own nest is not good eggs, play outside for a long time, the heart is also wild, more than half of the eggs will become bad eggs after the baby grows older (the testicles in the abdominal cavity have a high chance of cancer), and some bad eggs will spontaneously twist leading to abdominal pain performance. 3. Slipping away to play with the balls, the work is only done in general and the tunnel on the stomach is not completed, thus leading to the appearance of hiatal hernia (small intestinal gas); 4. Babies who go to kindergarten or elementary school and compare their balls with male babies of the same age only have one, which can lead to psychological changes, such as low self-esteem and ridicule. Treatment According to the 2014 American Urology Association (AUA) guidelines for the treatment of cryptorchidism, the only reliable treatment is surgery, and the recommended age for surgery is 6 months to 18 months, and previous hormonal treatments are no longer recommended. Depending on the location of the testicle, surgery may be performed in different ways. 1, the testicles are not accessible —- laparoscopic exploration; 2, the testicles are accessible —- traditional inguinal scrotal double incision surgery. Frequently Asked Questions 1.How to confirm that the baby is cryptorchid? Firstly, if the testicles are not palpable in the scrotum, you must be alert to cryptorchidism. Secondly, babies with cryptorchidism have poor scrotal development due to their own androgen secretion deficiency, so in children with bilateral cryptorchidism, the scrotum appears flat and wrinkle-free; in children with unilateral cryptorchidism, the scrotum appears flabby on one side. Parents can touch their baby’s scrotum while the baby is still in a hot bath. If the testicles can be touched in the scrotum at that time, but not in colder conditions, it is likely to be a retractile testicle. This is a physiological condition caused by higher excitability of the levator muscle and only requires regular annual review of the testicular condition. Of course, the safest way is to find a professional pediatric surgeon. 2.Do I need surgery for cryptorchidism? If the diagnosis of cryptorchidism is confirmed, surgery is necessary. Not only can it greatly reduce the chance of infertility of the baby in adulthood, but it can also reduce the chance of cancer of cryptorchidism in adulthood. 3.After the surgery, will the baby have normal fertility? Unilateral cryptorchidism will increase the risk of infertility. However, the chances of an adult with unilateral cryptorchidism having a baby of his own are very close to those of a normal man. Or, to put it another way, men without cryptorchidism are still infertile, and having cryptorchidism surgery just reduces the chance of infertility. (But no doctor can guarantee that if you have the surgery, you will be able to have children as an adult) 4. The name of cryptorchidism surgery is testicular fixation, which consists of two steps: testicular descent and fixation. According to the AUA guidelines, there is almost no possibility of descending after 6 months of age, and it is recommended that testicular fixation be completed within 1 year for babies after 6 months of age. Therefore, the best time for testicular fixation should be 6~18 months, under which the earlier the surgery, the better. 5.What are the complications after testicular fixation? First of all, the complications of testicular fixation are extremely rare. The common ones are retraction of the testicle into the abdominal cavity, blood leakage from the incision, and infection. 6.What should I pay attention to after the surgery? Avoid strenuous activities; do not sit on riding toys; review the status of testicles by ultrasound regularly every year.
