Firstly, the lesion sites are different; secondly, the pathogenesis and pathological patterns are different, which determines the treatment plan and prognosis of these two types of gastric cancer are also different. Generally speaking, since the lymph node metastasis pattern of upper gastric cancer and distal gastric cancer are different, distal gastric cancer can usually be treated by removing most of the distal stomach with some gastric tissues remaining; while the standard surgical treatment for upper gastric cancer is to remove all the gastric tissues. At the same time, the difference of surgical methods will make patients’ quality of life different, for example, patients with distal gastric cancer who have residual gastric tissues after surgery are prone to surgical complications such as gastroesophageal reflux. Overall, the 5-year survival rate of patients who can be treated by surgery for gastric cancer in China is 45%-50%. The average 5-year survival rate of patients with upper gastric cancer is 10%-15% lower than that of patients with distal gastric cancer, which means that the treatment effect of upper gastric cancer is worse or the prognosis is worse.
