At present, the incidence of digestive system tumors in China accounts for more than half of the total incidence of malignant tumors, among which gastric cancer, esophageal cancer and colorectal cancer rank 2nd, 4th and 6th respectively in the incidence of tumors, accounting for almost half of the incidence of all tumors. The number of deaths of esophageal cancer in China accounts for 46.6% of the deaths of esophageal cancer in the world. Clinical experts found that once diagnosed, patients are often in the middle or late stage of the disease. At this time, if treatment is carried out again, the curative effect is not good, and the 5-year survival rate is less than 20%. Not only is the patient’s life seriously threatened, but it also aggravates the economic burden of the patient and the family as well as the social burden, which leads to the return of poverty of many families due to the disease. In contrast, if esophageal cancer is diagnosed early and treated actively, the 5-year survival rate can reach more than 90%. The difference between early detection and late detection is very big: early esophageal cancer only needs to spend one to two million dollars to do an endoscopic surgery, and the 5-year survival rate is above 90%; while patients with middle to late stage only need five to eight million dollars for surgery, plus comprehensive treatment, at least more than 100,000 dollars. Take Jiangsu as an example, after investigation, the high incidence area of esophageal cancer in Jiangsu is mainly from Yangtze River to Huaihe River basin, such as Yangzhong, Taixing and other cities. The high incidence of esophageal cancer in this region is mainly related to the fact that people in this region generally like to eat “hot” food, and it is easy to burn or bruise the mucous epithelium of esophagus by eating too hot food, so that the mucous epithelium will be broken, ulcerated, and hemorrhaged, etc. If this is combined with smoking, drinking, and drinking alcohol, it will be difficult to treat esophageal cancer. If coupled with the repeated stimulation of bad habits such as smoking, drinking and pickled food, the mucosal epithelium will gradually become cancerous in the process of repeated proliferation and repair, resulting in a high incidence of esophageal cancer. In order to improve the cure rate of the disease in this region, in recent years, with the support of the national and local governments, a team of experts from our province has implemented early screening in this region. There are two types of screening commonly carried out at present, one is on-site census, mainly in townships and villages in the high incidence area, where gastroscopy census is carried out for asymptomatic people over forty years old on a village basis. The other is opportunistic screening, that is, in the outpatient clinic with upper gastrointestinal symptoms, such as nausea, vomiting, abdominal pain, acid reflux, eating discomfort and other symptoms of patients in a timely manner to do endoscopy. The early diagnosis rate of another most common malignant tumor, gastric cancer, is also very low. At present, stomach cancer accounts for the second place in the morbidity and mortality of malignant tumors in China. According to statistics, in 2000, there were 331,000 new cases of stomach cancer and 245,000 deaths in China, while in 2010, the figures had reached 436,000 and 323,000 respectively. Among the patients in China, early gastric cancer only accounts for 2-10%, with a 5-year survival rate of 95-97%, while about 85% of the patients with progressive gastric cancer can receive surgery, but the 5-year survival rate is only 20-30%. What is more worrying is that there is a tendency of rejuvenation of gastric cancer, rough statistics show that about 15% of the diagnosed patients are young people under 40 years old.
