Treatment of insomnia from the theory of qi lifting and lowering

Insomnia is caused by the disharmony of yin and yang, so it is important to regulate the ascending and descending of the meridians to promote the smooth circulation of qi and blood and the secretion of yin and yang by lowering the phase fire to draw yang into yin, opening the sun to traffic the heart and kidneys, and consolidating the middle jiao to smooth the flow of qi and qi in the treatment of insomnia.  1.The relationship between the theory of meridian ascending and descending and insomnia The six meridians are the theoretical core of “The Treatise on Typhoid”, in which the treatment of insomnia is scattered, but there are still many prescriptions for the treatment of “insomnia” in the six meridians. Throughout the present-day Chinese medical treatment of insomnia, it often starts from the function of the internal organs, often ignoring the impact of the circulation of the meridians that run the qi, blood, yin and yang on the disease.  Man and nature are small universes that correspond to each other, and the yin and yang of the natural world, as well as the rise and fall of the body, have an effect on the human body, as said in the “Ling Shu? The mouth question says: “When the yang energy is exhausted and the yin energy is strong, the eyes will close”. As a normal physiological process, sleep is the most direct reflection of this feature of the correspondence between yin and yang of the human body and the outside world. As the ancients said, “A yin and a yang is called the way, and a yin and a yang bias is called a disease.” Therefore, the Nei Jing establishes that the root cause of insomnia is the “disharmony of yin and yang”, and that the smooth operation of the twelve meridians, which distribute the qi and blood of yin and yang, is the most direct manifestation of the harmony of yin and yang.  Ling Shu? Wei Qi” as it is said: “Yin and Yang follow each other, the outer and inner coherence, such as the endless ring.” The relationship between the circulation of the twelve meridians and yin and Yang is highlighted in its lift movement, and has a certain pattern: where the hand yin and Yang meridian gas up, with the name of the foot yin and Yang meridian gas is down; where the hand yin and Yang meridian gas down, with the name of the foot yin and Yang meridian gas is up, and the twelve meridians are arranged according to the order of the circulation of the lift connection. Because of the importance of the ascending and descending mechanism of meridians in the development of insomnia, and according to the viewpoint of the ascending and descending of meridians, it is known that meridians have the characteristic of being connected with each other, so that their circulation process has descending and ascending in ascending. Therefore, the use of a single upward or downward medicine is often ineffective, so the actual clinical practice should be based on the order of meridian connection and lift, and more need to take into account the function of the middle jiao as the hub of qi. Clinical use of drugs also need to pay attention to the lifting and lowering of drug properties, to the meridian, Li Dongyuan summarized it as: “thin taste is ascending, thin gas is descending, thick gas is floating, thick taste is sinking.” It can be seen that for the taste of the drug, according to the disorder of the meridians of insomnia patients to identify the evidence to choose the drug, often receive a more certain effect. The practical application can often be started from the following aspects: 2.1, lowering the phase fire and lead Yang into Yin The fire of the five elements has a gentleman fire phase fire, so corresponding to the six meridians of the three yin and three Yang. The gentle fire refers to the heart fire, and the phase fire is often said to be Shaoyang phase fire, but the liver, gallbladder, kidney and pericardium are all involved in the phase fire, and the vital fire is also a phase fire. Under physiological conditions, Shao-Yin Jun-Huang should ascend, Shao-Yang Xiang-Huang should descend, and the Xiang-Huang descends and returns to its original position, “hair and all in the section”, while under pathological conditions, it changes into evil fire, that is, the so-called “Xiang-Huang is the thief of Yuan Qi”. It can be seen that sleep, as a normal physiological process, also lies in the return of the phase fire to its original position, thus inducing Yang Qi into Yin and sleep, but nowadays clinical insomnia patients commonly have Yin deficiency and Yang is not attached, water deficiency is not able to control the fire, resulting in the phase fire of Shaoyang Bile meridian or the heart pericardium of Conjunctive Yin does not descend but reverses. Symptoms include coldness in the lower extremities, diluted stools, increased urination, redness of the face, irritable heat in the palms and other manifestations of upper heat and lower cold.  For example, if the phase fire of syncopal pericardium does not descend, Chai Hu can be given to ascend the Qi of Shaoyang Sanjiao meridian, and leek, Cistanches, Bacopa monniera, Huang Jing, etc. to replenish Shaoyin kidney water; and if the phase fire of Shaoyang Bile meridian does not descend, in addition to ascending the Qi of Sanjiao, it also needs to ascend the Qi of Conjugate Yin Liver meridian and descend the Qi of Tai Yin Lung meridian, etc., the prescription can be chosen from peony to descend Shaoyang Bile meridian; orris and almond to descend the Qi of Tai Yin Lung meridian; and Angelica sinensis, Dihuang, Aconite, etc. can be given to nourish blood and moisten wood, Chuanxiong, Gui Zhi, etc. The Chinese rhizome, Chuanxiong and Gui Zhi can be used to ascend the Liver meridian of Conjunctive Yin. In this way, the front and rear channels of the meridians dominated by the phase fire are raised and lowered, so that the phase fire can be stopped and the yang energy can be induced into the yin, and sleep can be restored. In the Treatise on Typhoid Fever, Sour Jujube Soup and Xiao Jianzhong Tang also lower the phase fire from two different cases of cold and heat in Shaoyang Bile meridian respectively.  2.2, open the sun and traffic heart and kidney hand Shaoyin heart meridian down, foot Shaoyin kidney meridian gas rise, the two gas connected to sleep, but the connection between the heart and kidney meridian need to rely on the sun meridian, the cardinal doctrine that the sun is the main open, Yang Qi from the rise; and the hand sun meridian belongs to the small intestine, foot sun meridian belongs to the bladder, the small intestine is the main liquid, the bladder and the state capital of the official, the water stored in the cold nature of Yin, so the sun cold water and need to descend. It is clear that the Sun meridian has the characteristics of “ascending and descending”, “to open for descending”. If the Sun meridian qi is not smooth, when open and not open naturally cause when descending not descending, not only appear Xiang back discomfort, back head swelling and pain, sweating or no sweating and other signs of the Sun meridian evidence of disharmony, but also often affect the intersection of the heart and kidney qi sleeplessness, and this type of evidence patients are often seen with heart panic, chest tightness, night rise more often, fear of cold and other manifestations. In addition to using Scutellaria baicalensis, Huang Lian and Chuan Niu Kne to lower the fire of the heart meridian, and Gum and Chicken Zi Huang to benefit the kidney water to help the qi of the kidney meridian to move up, we should pay more attention to the Sun meridian, which is the main channel for the heart and kidney to rise and fall, and open the Sun to lower the fire of the heart, so the prescription can be chosen to open the Sun and assist the intersection of the heart and kidney meridians. Just as the famous doctor Liu Du Zhou in the treatment of deficiency labor caused by the heart and kidney do not intersect, Yin and Yang to hold the evidence of unfavorable in Zhang Zhongjing “Jin Kui Essentials. If the Yang Ming Stomach meridian does not descend due to poor diet, overwork, mental stimulation, etc., and the Qi of the internal organs does not flow, and at the same time affects the ascending and descending of the Tai Yin Spleen meridian, the clear Qi does not ascend, which can lead to “restlessness if the stomach is not in harmony”, and patients with this type of symptoms mostly have dizziness, heavy eyelids, dry stools and other manifestations. Therefore, treatment often requires consolidation of the middle Jiao and regulation of elevation. Clinically, it is also necessary to change according to the symptoms. If there are symptoms such as abdominal fullness, dullness, loose stools, and weakness in the province, we can often give round-grained rice, jujube, dang ginseng, and dry ginger to supplement the Taiyin spleen meridian and help to raise the clear; if there are symptoms such as constipation and acne, we can also give half-xia, rhubarb, peach kernel, and safflower to diarrhea the Yangming stomach meridian and help to pass down. The treatment also requires a combination of lifting and lowering to be effective.  Because the spleen and stomach are located in the middle jiao, they are like the axis of qi movement, providing a pivot for other meridians to rise and fall. If there is no evidence of discomfort in the middle jiao of the spleen and stomach as mentioned above, it is often necessary to consolidate this major channel of communication in the middle jiao and facilitate the ascending and descending of the meridians, such as in the case of “Dioscorea Pill” in the “Treatise on Typhoid Fever”, where the formula of Rizhong Tang is given to stabilize the phase fire and help it to return to its place. In “Ma Huang Sheng Ma Tang”, we use the formula of Si Jun Zi Tang to tonify the spleen and stomach, to help open the fire of the lung meridian and raise the qi of the sympathetic yin, which is also often needed to help the two qi connect when communicating with the heart and kidney.  These aspects described above are only three starting points for regulating insomnia from meridian ascending and descending, and there are still many meridian qi movement links to start interventions. As long as the general outline of ascending with descending, ascending and descending in balance, and drawing yang into yin is grasped, unexpected effects can be received in the treatment of insomnia.
