Cast stones can usually be managed with a tuboscopy but with holmium laser lithotripsy. A cast stone means that the tube inside the kidney is filled with stones, and some cast stones form antlers also known as staghorn-shaped stones. The hosecope, also known as a flexible ureteroscope, is used primarily for the treatment of upper ureteral stones and kidney stones smaller than 2 cm. After a safety guidewire is placed through the urethra into the ureter, a flexible mirror sheath is placed under the guidance of the guidewire, and then the flexible ureteroscope is placed under direct vision. The flexible scope can follow the guidewire into the renal pelvis or calyx and locate the stone. Holmium laser is then applied to pulverize the stone into tiny fragments that can be easily discharged, and the stone is removed using a flexible tube scope.
