Many years have passed, and as a surgeon, the passion and perseverance that I had when I was young have been unknowingly abandoned in success and frustration, but this request from my patients has always been unforgettable to me. At the end of the last century, with the accumulation of experience in laparoscopic surgery and the continuous improvement of instruments, the answer to this question was provided. Soon, a set of laparoscopic surgical instruments came into our hospital as well. In 2003, after returning from a study trip, we finally performed our first laparoscopic thyroid surgery successfully. In fact, laparoscopic thyroid surgery is not a mystery. Simply put, it is a method of removing the thyroid gland using laparoscopic technology. The main procedure is to make two to three small surgical incisions, only 5-10 mm in diameter, in a hidden part of the chest, and then to separate the thyroid gland in the neck through the skin and inject carbon dioxide gas to create an artificial operating space under the skin in the anterior chest and neck. The operating instruments are then placed, and the thyroid and its tumor are removed through direct laparoscopic vision and magnification using advanced ultrasonic knife cutting techniques. The patient is discharged from the hospital 3 to 5 days after surgery. Laparoscopic thyroid surgery is a new technique developed on the basis of laparoscopic surgery. The emergence of this surgical method can be considered as a technical revolution in the history of surgery. It changes the history of traditional surgical procedures that required a direct surgical incision at the site of the neck lesion. Instead, a small incision is made away from the surgical site and the operation is performed with the help of slender surgical instruments. In this way, surgical scars are not left on the patient’s exposed body parts, and the scars that were left on the neck are reduced and hidden in a hidden place on the body, so that while having a safe and effective clinical treatment effect, the aesthetic effect of no surgical scars on the neck is achieved, meeting the cosmetic requirements of women who love beauty. Due to the complex and delicate local anatomical relationship of the thyroid gland, and its proximity to important organs such as the trachea, esophagus and large blood vessels in the neck, as well as important nerves around it, coupled with the fact that this surgery is a percutaneous operation, the surgical space is small. This requires the surgeon to have not only extensive experience in traditional thyroid surgery, but also skilled endoscopic surgery techniques. Therefore, it is a new challenge for head and neck surgeons. There are many different routes for lumpectomy thyroid surgery, among which the transareolar anterior thoracic route is currently one of the most desirable and commonly used methods. Of course, lumpectomy thyroid surgery does not deal with all thyroid disorders. In general, lumpectomy thyroid surgery is indicated for: (1) simple goiter where conservative treatment has failed; (2) nodular goiter or cystic hyperplasia; (3) thyroid adenoma; (4) primary or secondary hyperthyroidism up to grade II, and optionally for differentiated thyroid cancer with a small primary focus. Partial thyroid lobectomy, lobectomy, subtotal thyroidectomy, bilateral subtotal thyroidectomy, total thyroidectomy and lymph node dissection in the central region of the neck have been performed in clinical practice.
