If thyroid hormone levels are elevated, thyrotropic hormone levels should be decreased, which is consistent with hyperthyroidism, and should be combined with thyroid ultrasound and thyroid hormone receptor antibodies to further confirm the diagnosis of combined primary hyperthyroidism. If thyroid hormone levels are decreased, thyrotropin levels are significantly elevated, consistent with hypothyroidism, and need to be interpreted in conjunction with levels of thyroglobulin antibodies and antithyroid peroxidase antibodies. If both antibody levels are significantly elevated then chronic lymphocytic thyroiditis in combination with primary hyperthyroidism is considered, and appropriate medication should be administered.
