Weight loss and sugar reduction, why weight loss surgery can reduce sugar

At the World Diabetes Congress, Professor Mingrone from Italy presented the results of a new study that early weight loss surgery in patients with type 2 diabetes is effective in improving blood glucose levels, even in patients with a body mass index <30kg/m2. The remission rate of diabetes was found to be 14.4% vs. 63.7% (p<0.001) in the medication and bariatric surgery groups, respectively (remission criteria: fasting glucose <5.6mmol/L). As we can see, the remission rate of type 2 diabetes by weight loss surgery is 4.5 times higher than that by medication! So, through the experiment we can effectively know that weight loss surgery can effectively lower blood sugar. With the huge number of diabetic patients in China and the effectiveness of weight loss surgery so obvious, can they all go for this surgery? The answer is no. There are strict surgical indications for weight loss surgery, and the Chinese Guidelines for the Surgical Treatment of Obesity and Diabetes, published in November 2014, have strict requirements for patients' age, body mass index, islet function and other indications for diabetes surgery." It is understood that the indications regarding diabetes surgery include the following: 1. age Patients with diabetes whose bodies have not fully developed or who are over 65 years old cannot undergo surgery. Body Mass Index (BMI) Body Mass Index (BMI) (weight divided by height squared) is an indicator used to define "obesity" in medicine, and is also an important reference indicator for diabetes surgery, and the corresponding surgical classification differs for different BMI. When BMI≥32.5, active surgery is recommended; when BMI is 27.7~32.5, surgery is recommended; and when BMI is 25~27.5, we suggest that surgery should be carried out carefully. 3. Pancreatic islet function The duration of type 2 diabetes cannot be greater than 15 years, and the pancreas should ensure a certain insulin secretion function, and the fasting serum C-peptide should not be less than half of the lower limit of normal value. If the pancreas does not have any "self-care ability", there is no point in surgery. Why do we need to meet so many requirements to perform diabetes surgery? Because only by strictly following the surgical indications can we ensure that the patient will receive the best possible treatment. In the eyes of the surgeon, surgery is aimed at improving and enhancing quality of life, and if the evaluation predicts that the patient will not achieve the desired outcome, such surgery may as well not be done. For patients with obese diabetes, weight loss alone does not solve the problem of high blood glucose; the best way is to lose weight and lower blood glucose at the same time. early weight loss surgery for type 2 diabetes can be effective in improving blood glucose levels, even in patients with a body mass index <30 kg/m2.
