II. Etiology
1, caused by herpes simplex virus infection, mainly herpes simplex virus II (HSV-II), but currently the chance of genital herpes caused by herpes simplex virus I (HSV-Ⅰ) infection is significantly higher.
2. Triggering factors: fever, cold, physical and mental fatigue, depression, excessive alcohol consumption, menstruation, etc.
Three, the route of infection
1, sexual contact transmission: mainly seen in sexual promiscuity or multiple sexual partners.
2, placental transmission: that is, mother-to-child transmission.
3, the birth canal transmission.
4, other, less common, low probability of occurrence.
Including the following.
(1) blood transfusion infection, drug infection;
(2) indirect contact transmission, such as a small number of patients with syphilis spirochetes underwear, bedding, towels, razors, toilets, etc..
Four, classification
1.Primary genital herpes;
2.Recurrent genital herpes
This refers to the recurrence of primary genital herpes within 1-4 months after the lesions have subsided, and the recurrent infection usually occurs at the original site;
3.Subclinical genital herpes
The patient with asymptomatic genital herpes, but with positive serological tests.
V. Clinical manifestations
1. Incubation period: 3-14 days.
2. Site of lesions: external genitalia or anus, mostly in the penis, coronal sulcus, glans prepuce in men; in the labia majora and minora, mons pubis, clitoris, uterus, etc. in women.
3, skin lesions: small corn or green bean size blisters blisters, scattered distribution or gathered in piles, 2-4 days to form erosion or ulcers.
Note: Many patients only show vesicles or ulcers or even only painful sensation without skin lesions when they visit the clinic. However, we cannot easily exclude genital herpes.
4. Course of disease: 7-10 days to heal spontaneously.
5.Self-perceived symptoms: pain. If it occurs in the anus, it can be manifested as anorectal pain, constipation, increased discharge, etc.
6.Prognosis: good, but easy to recur.
VI. Diagnosis
1.History of unclean sexual intercourse or spousal infection.
2.Clinical manifestations (as above).
3, Laboratory tests: HSV-Ⅱ-IgG or HSV-Ⅰ positive (suggesting infection or previous infection) HSV-Ⅱ-IgM or HSV-Ⅰ positive (suggesting acute infection period).
VII. Treatment
1.Objective
To shorten the course of the disease, prevent recurrence, and reduce the number of recurrence
2.Treatment
(1) Primary disease
① Systemic treatment
Anti-viral treatment, such as acyclovir tablets or valacyclovir tablets, etc. for 7-10 days. At the same time, give immunity-boosting treatment, such as pidomod oral solution, transfer factor oral solution or injection, thymidine capsule, BCG injection, etc., to prevent recurrence.
②Topical treatment
Give acyclovir ointment or ganciclovir ointment, etc.
(2) Relapsing type
(1) Systemic treatment
Anti-viral treatment, such as acyclovir tablets or valacyclovir tablets, etc. for 5 days. At the same time, give immunity-boosting treatment, such as pidomod oral solution, transfer factor oral solution or injection, thymidine capsule, BCG injection, etc. to prevent recurrence.
②Topical treatment
Give acyclovir ointment or ganciclovir ointment, etc.
3.Frequent type
It refers to more than 6 times a year or more than 2 times a month. Suppressive therapy (omitted) can be given, together with traditional Chinese medicine, which has good effect.
Prevention and education
1. At the time of onset, abstain from spicy and stimulating food, drink less alcohol, strong tea and coffee, avoid physical and mental fatigue, take more rest and keep a happy mood.
