In clinical practice, people often ask how to treat these patients as oncologists, since cancer is a terminal disease. First of all, it depends on how to define “terminally ill patients”. Is it a terminal disease as long as cancer is considered by the general public, or is it a terminal disease because there is no effective treatment for some advanced tumors? The treatment options and objectives for these two cases are very different. With the advancement of technology, including drug development, surgical concepts and techniques, many tumors that were once considered “terminal” have a high cure rate. It does not mean that cancer is a terminal disease. Therefore, the concept should be changed. The earlier and more standardized the treatment, the better the treatment effect and the higher the cure rate. Don’t take it for granted that there is no meaning of treatment, which is harmful to people and themselves. Another kind of terminal disease is the advanced stage of tumor, indeed there is no effective treatment plan, and the patient is already in the end stage of the disease. At this time, the treatment should not be anti-tumor therapy, but to alleviate the patient’s pain, such as relieving pain, which can be done by actively applying painkillers, up to the best effect of being pain-free. At this stage, it is almost meaningless to give any more radiotherapy, chemotherapy or immunotherapy. It is also not tolerated by the patient. At this stage, the patient and family are fully informed of the condition, and the change in treatment modality. The current treatment of tumors is indeed a dilemma. The treatment of tumors, in general, is more expensive. Sometimes, it does not necessarily mean that if you spend money, you will have a good result. Especially for advanced stage patients, the treatment is not only a matter of treatment, but also a matter of social, family, various relationships, and more importantly, economic basis. As an example, for patients with liver metastases from gastric cancer, if the patient’s tumor is strongly positive for Her-2, chemotherapy can be combined with Herceptin targeted drug therapy, and some patients (the patients I treated) responded particularly well, and after 3-4 cycles, 10 cm of liver metastases almost disappeared, and finally the primary foci were almost inactive, so that the patient’s survival was greatly extended. However, this does not mean that all patients with liver metastases from gastric cancer, and who are HER-2 positive, have such an effect. Targeted therapy is more expensive and self-paying. At this point, if a such patient with average financial condition comes to the clinic, suitable for targeted therapy, but the response effect after treatment is unknown, and clearly tells you that the family condition is average. As a doctor, how do you tell the patient and family? Isn’t it, difficult. Do not say it, in case, very effective, you delayed the treatment of others; say it, the family conditions do not allow, if said in front of the patient and the child, the patient and the child’s heart is difficult, the patient wants to treat, but also know the family situation, add burden to the child, sometimes can only condescend to say, no need. The child’s heart is even more difficult, because the conditions are difficult and cannot give the parents good treatment. Generally this situation, I will give the family advice, a comprehensive consideration, if still possible, tight hand, with one or two cycles, the cost will not be too high, to see the effect and then decide the next step in treatment. There is an effect, even if you borrow money, but also to do filial piety, more than a year with their children, is how great happiness. If there is no effect, it is decisive to stop, not to spend unnecessary costs, have tried their best, no regrets, after all, people can not win the day. As for extending the treatment for several years, it is not meaningful. It depends on how to understand, the treatment of tumor, not to mention a few years, is to extend a year, is a good treatment plan. For the old people who want to see their children get married, it is meaningful to him/her to prolong January and see their children get married; for some patients who have some unfulfilled wishes, it is meaningful to prolong even half a year, March for him/her, to finish it; for children with young children, it is valuable to stay with them for even one more year, half a year, to grow up. For life, no one can easily make the decision for others. But, reality often makes people powerless to spit. Cherish life, love your body, for yourself and for those who love you more!
