Abstract: A 15-year-old girl accidentally caught a cold during her menstrual period, which led to a decrease in body resistance, coupled with a long menstrual flow, and did not change the sanitary napkin normally when the volume was low in the late menstrual period, resulting in vulvar itching, which the patient could not resist scratching, leading to epidermal rupture and vulvar pain, and was diagnosed as acute vulvovaginitis at the hospital, where she was given symptomatic treatment with medication to control her condition and relieve her symptoms.

I. Initial consultation
The patient is a 15-year-old girl with irregular menstruation, menstrual period of 7-10 days/time, menstrual cycle of 30-50 days, last menstrual period on May 22, 2022, the second day of menstruation, the patient accidentally got wet with rain and had cold symptoms. On May 28, she developed mild vulvar itching, and on May 30, her menstrual period cleared, but the vulvar itching worsened, and she couldn’t help scratching her vulva, which led to vulvar pain. The outpatient gynecological examination showed that the vulva and labia were obviously red and swollen, and some of the epidermis of the vulva showed scratches and skin rupture.
II. Treatment history
The patient and her mother explained her condition to the patient. The patient’s menstrual period and the influence of the flu led to a decrease in body resistance, and the sanitary napkins used were not changed in time, resulting in acute vulvovaginitis. Mupirocin ointment is applied to the vulva alternately to promote the healing of broken skin and treat mycobacterial infections. At the same time, patients should be instructed not to scratch the vulva, pay attention to keeping the vulva dry and clean, rest in bed as much as possible, avoid excessive activities that cause friction on the affected side of the vulva, and do not eat stimulating food.
III. Treatment effect
The patient was advised to continue the original treatment for 5 days and stop the medication after the symptoms had completely improved. The patient was advised to stop the medication and to change the sanitary napkin in time.
IV. Notes
We are glad that the patient’s discomfort was effectively relieved after timely treatment. Since the patient is young and suffers from acute vulvodynia, she may have psychological fears. It is recommended that her family members show concern and explain more about female hygiene to help overcome psychological fears, understand the disease correctly, and know how to prevent recurrence.
Patients are advised to buy regular brands of sanitary napkins and to change them regularly during menstruation to keep the vulva dry and clean to reduce the occurrence of vulvodynia and vaginitis. Diet should be less spicy, greasy and inflammatory food and more light food, vegetables and fruits, which is more helpful to reduce the recurrence of vulvodynia.
V. Personal Insights
Teenage girls are generally less likely to have vulvovaginal inflammation. This patient is due to the fact that when she meets a cold during her menstrual period and her body’s resistance is significantly reduced, she does not pay attention to her personal hygiene, which induces infection and leads to acute vulvovaginitis. In order to prevent vulvovaginal infections, it is important to buy disinfected sanitary napkins during menstruation, and to change the sanitary napkins during menstruation, which should be used for no more than 4 hours.
