In China, the incidence of reflux esophagitis (RE) has gradually increased as people’s standard of living has improved and their diet has changed, with an increase in fatty foods. This is precisely why GERD is more common in Western countries. Gastroesophageal reflux disease (GRED) is divided into three separate types: erosive esophagitis (EE), non-erosive reflux disease (NERD) and Barrett’s esophagus (BE). Non-erosive GERD (NERD) accounts for about 70% of GERD (GRED). The severity of clinical symptoms in patients with NERD is similar to that of EE, but treatment with proton pump inhibitors (PPI) and others is less effective. The pathogenesis of NERD is not well understood and may be caused by acid-base (gastric acid, bile) irritation of the refluxed material, by increased sensitivity of the extrapyramidal receptors, or by abnormalities in central nervous system processing. The latter two forms may be somatic manifestations of psychosomatic disorders, where abnormalities in perceived symptoms are amplified in states of psychosomatic abnormalities (anxiety, depression, etc.) to form functional heartburn, or where autonomic stress induces a transient relaxation of the lower esophageal sphincter (LES) leading to the development of reflux. Modern medical research has found that most patients with NERD have varying degrees of psychosomatic abnormalities, and during psychological stress, gastric motility is significantly reduced and gastric emptying is delayed, and psychological factors can affect esophageal sphincter pressure and gastroesophageal motility through neurological and gastrointestinal hormone mediations. And the quality of life of patients is significantly reduced compared to healthy individuals, even worse than patients with chronic organic diseases. Whether it is PPI, gastric mucosal protective agent, or pro-gastric motility drugs for NERD, they all target the relief of reflux symptoms after they appear and cannot address the abnormal visceral sensation and underlying psychosomatic abnormalities to play a role in the bottom of the barrel. Therefore, while treating NERD patients with anxiety and depression with traditional anti-NERD drugs, it is of deeper clinical significance for NERD patients with anxiety to be treated with additional anti-anxiety drugs and supplemented with psychotherapy. Dextran is a compound preparation for the treatment of psychiatric disorders, which is effective for anxiety and depression and helps to improve the dysfunction of the lower esophageal sphincter, and also effectively improves the mental perception abnormalities. Therefore, for patients with refractory NERD, clinicians should be vigilant and pay attention to the presence of psychosomatic symptoms when taking medical history (patients should be tested using anxiety and depression scales if necessary) in order to give patients more timely and reasonable treatment plans to maximize their symptoms and improve their quality of life.
