Is chronic pain itself a disease?

Pain is an undesirable feeling that everyone will experience in their lifetime, and with the development of modern medical concepts, pain has been considered the fifth most important vital sign after breathing, pulse, blood pressure and body temperature. In particular, chronic pain is a disease in itself, which can seriously endanger the physical and mental health of patients, and even endanger their lives, and requires active diagnosis and treatment. What is pain medicine? Pain medicine is an emerging and marginal discipline that focuses on the diagnosis and treatment of various types of chronic pain, integrating neuroscience, anesthesiology, interventional radiology and pain-related branches of orthopedics, and is an important part of anesthesiology. In 2004, the World Academy of Pain Research (IASP) designated the third week of October as “World Pain Day”. In China, the Chinese Medical Association’s Pain Society has designated the week following World Pain Day as “China Pain Week”. In particular, in 2007, the Ministry of Health added “pain medicine” as a clinical subject in the “List of Medical Institutions”, and stipulated that hospitals above the second level must have pain clinics or pain departments, marking the pain treatment business as an important part of the evaluation of the level of hospital health services. What diseases are treated in pain clinics? The scope of pain medicine is very broad, mainly for the treatment of chronic pain diseases, and its treatment scope mainly includes: Headache: migraine, cervicogenic headache, myotonic headache, etc. Neuralgia: trigeminal neuralgia, postherpetic neuralgia, diabetic neuralgia, neuropathic pain, pain after nerve injury, central pain Herniated disc: cervical disc herniation, lumbar disc herniation Lumbar disc herniation Bone and joint pain: lumbar and leg pain, cervical spondylosis, knee osteoarthritis, heel pain, temporomandibular joint dysfunction, degenerative osteoarthritis, etc. Tissue pain: tenosynovitis, frozen shoulder, tennis elbow, soft tissue injury, lumbar muscle strain, acute and chronic lumbar sprain, lumbar back muscle fasciitis, etc. Cancer pain: advanced cancer pain, bone metastatic pain. Non-painful diseases: intractable eruption (hiccups), acute facial neuritis, plant nerve dysfunction, etc. Anesthesia consultation: anesthesia evaluation and post-surgical analgesia. What are the specialty treatments for pain? There are a wide range of pain treatment methods. In addition to traditional medication and physical therapy, various minimally invasive treatment methods relying on imaging guidance are its unique treatment features. Nerve block is the most common method, which is highly effective, safe and economical, especially for patients who are not yet suitable for surgical treatment due to the ineffectiveness of drug treatment. Unlike the so-called “closure”, nerve block essentially belongs to the category of minimally invasive interventional treatment, which is to clearly diagnose the patient’s disease through modern examination methods, and locate the lesion through X-ray, CT, MRI and other imaging methods, and inject anti-inflammatory and analgesic drugs accurately into the vicinity of the diseased nerve to eliminate the nerve inflammation and edema, thereby achieving pain relief. The purpose of pain relief is thus achieved. The use of medical triple oxygen (O3) is another important new treatment technology in the pain clinic. Triple oxygen has a strong anti-inflammatory and analgesic effect and can also be used to dissolve the herniated nucleus pulposus, which is safe and efficient, and is effective in the treatment of painful diseases such as herniated disc, cervical spondylosis, knee osteoarthritis, postherpetic neuralgia and diabetic foot. Recently, the pain clinic has also tried to carry out radiofrequency thermal coagulation technology, and has made technical breakthroughs in target thermal coagulation for disc herniation and ganglion thermal coagulation destruction for trigeminal neuralgia, with the aim of gradually providing inpatient minimally invasive surgical treatment services for the majority of patients. The principle of pain treatment is different from what people often call “closure”, which blocks the nerve conduction pathway of nociception. The vicious cycle of “pain → muscle tension or small blood vessel smooth muscle spasm → local ischemia → tissue hypoxia, metabolite accumulation → increase in painful substances → nerve plasticity reaction → increase in pain” is blocked. It reduces sympathetic excitability, dilates blood vessels, and improves circulation and metabolism. Anti-inflammatory effect, eliminating local non-bacterial and immune neuroinflammation. Improving patients’ emotions, regulating psychological state, raising pain threshold and relieving the compression of nerves by cervical and lumbar intervertebral discs From scratch, from weak to strong Since its establishment, the pain clinic has been adhering to the concept of “removing pain for the people” and striving to become a small specialty clinic with outstanding pain diagnosis and treatment characteristics. With the support of hospital leaders at all levels, the clinic now has a sterile treatment room of high standard and has purchased more advanced treatment equipment such as triple oxygen therapy instrument, and the hardware equipment has reached the leading level in China. At present, the backbone physicians of the pain clinic are Guan Yuan and Pang Xiaolin, who first studied under Professor Yan Xiangmo, who is known as the “father of Chinese pain medicine”, and then studied under Professor Ni Jiachao, the president of the World Pain Association, for a master’s degree in pain. Through years of clinical practice, they have a solid professional foundation and have accumulated rich clinical experience, and have outstanding specialties in the treatment of neck and lumbar pain, knee pain and cancer pain.
