Clinical advantages of Chinese medicine in the treatment of coronary heart disease

At present, there are many clinical evidences showing that Chinese medicine has certain clinical advantages in relieving angina pectoris, intervening in restenosis after coronary intervention (PCI), preventing and treating no-reflow after reperfusion, regulating arrhythmia, improving heart failure and secondary prevention of myocardial infarction, etc. 1. 1.Ease angina pectoris Angina pectoris is caused by insufficient blood supply to coronary arteries due to coronary artery atherosclerosis stenosis and temporary ischemia and hypoxia of myocardium, which is the most common clinical manifestation of coronary heart disease. Chinese medicine can achieve the purpose of relieving angina pectoris by expanding coronary artery, reversing coronary artery plaque and promoting neovascularization. The results of the systematic evaluation of the study on the treatment of angina pectoris by Compound Danshen Fang show that, compared with nitrate esters (elimination of cardiac pain), Compound Danshen Fang can effectively alleviate the symptoms of angina pectoris, improve the ischemic electrocardiogram and the index of blood rheology, regulate the blood lipids, and alleviate the concomitant symptoms; in the aspect of safety, Compound Danshen Fang has fewer adverse events, and does not have vasodilatation headache, flushing, vertigo, postural hypertension, reflex tachycardia, and reflex tachycardia, as in the case of nitrate esters. Hypotension, reflex tachycardia and other adverse reactions, better tolerated. Compound Danshen Dropping Pills successfully passed the phase II clinical trial of the U.S. Food and Drug Administration in 2010, and the results of the trial suggest that the drug has anti-anginal effects at both high and low dose levels, and the number of angina episodes in the high dose group is significantly lower than that in the low dose group and the placebo group. Compound Danshen Dropping Pills can effectively relieve angina symptoms in patients with coronary heart disease, improve ischemic electrocardiogram, reduce cholesterol and triglycerides, and no obvious adverse reactions were found. Intervention of post-PCI restenosis Excessive proliferation of vascular endothelium after PCI may lead to restenosis in the stent. At present, modern medicine adopts antiplatelet drugs, drug-eluting stents and other means to prevent and control post-PCI restenosis, and certain progress has been made, but most patients cannot escape the fate of coronary artery restenosis despite the long term oral antiplatelet drugs and intensive lipid-lowering treatment after operation. Current research shows that Chinese medicine can act on multiple pathologic links of restenosis, and has certain efficacy advantages in the prevention and treatment of postoperative PCI restenosis. Academician Chen Keji believes that blood stasis is closely related to post-PCI restenosis, and clinical intervention studies using blood stasis soup preparation have confirmed that the drug can reduce the recurrence of angina pectoris in patients with coronary heart disease after PCI, and has a certain effect on the prevention of post-PCI restenosis; domestic scholars have added Tongxinluo capsule on the basis of conventional dual antiplatelet therapy for the treatment of post-PCI restenosis, and the results suggest that adding Tongxinluo capsule can reduce stent stenosis, and the results suggest that adding Tongxinluo capsule can reduce the recurrence of stent stenosis. The results suggest that the addition of Tongxinluo capsule can reduce the incidence of restenosis after stent implantation. The results showed that the addition of Tongxinluo capsule could reduce the incidence of restenosis after stent implantation. It was also observed that conventional treatment with Musk Cardioprotection Pill was significantly better than conventional treatment in relieving angina pectoris and improving restenosis after PCI. Ren Yi et al. conducted a systematic evaluation of randomized controlled trials of traditional Chinese medicine to prevent and treat restenosis after PCI, and the results showed that the incidence of restenosis and the recurrence rate of angina pectoris in the group of traditional Chinese medicine combined with conventional Western medicine were lower than those in the group of conventional Western medicine alone. 3, prevention and treatment of reperfusion after no reflow After blood flow reconstruction vascular recanalization, there are still about 25% of patients will occur myocardial tissue no reflow and slow reflow phenomenon. The integrity of coronary microcirculation and adequate tissue perfusion is the guarantee of successful reperfusion, and the complication of no-reflow after reperfusion treatment has become a new clinical problem. The results suggest that multiple pre-administration of compound danshen drip can prevent ischemia-reperfusion-induced cardiac microcirculatory disorder and myocardial injury in rats, while post-administration of compound danshen drip after reperfusion not only reduces the cardiac microcirculatory disorder and myocardial injury in rats, but also inhibits the occurrence of myocardial fibrosis. Tongxinluo on acute myocardial infarction PCI after myocardial non-reflow prevention and control of clinical research shows that Tongxinluo can significantly improve the 24h post-PCI electrocardiogram ST-segment fallback amplitude and complete fallback rate, significantly increase myocardial blood perfusion, thus improving cardiac function; and unstable angina pectoris patients taking Tongxinluo capsule nail fold microcirculation changes in the clinical observation shows that Tongxinluo capsule after treatment of the patient’s nail fold A number of indicators of microcirculation improved significantly compared with the pre-treatment.
