How much the prognosis after myeloma autotransplantation is related to upfront chemotherapy

The prognosis after autologous transplantation for myeloma is closely related to pre-chemotherapy, and pre-chemotherapy is essential. 1. For myeloma patients who are suitable for autologous hematopoietic stem cell transplantation, induction therapy containing new drugs + hematopoietic stem cell transplantation should be used as much as possible. At present, induction therapy is mostly based on three-drug combination of proteasome inhibitors (e.g. bortezomib), immunomodulators (e.g. thalidomide) and dexamethasone, and the combination of these three drugs is better than that of two drugs. 2. Autologous hematopoietic stem cell transplantation can improve the remission rate, so that patients are in long-term remission, and the survival period can be significantly extended. And pre-treatment with chemotherapy induction before transplantation can kill tumor plasma cells in large quantities, reduce tumor cell load and make patients reach remission state, thus ensuring the safety and success rate of transplantation, and then prolonging the survival time and improving the prognosis. Before myeloma autologous hematopoietic stem cell transplantation, chemotherapy induction should be carried out in a standardized and sufficient course of treatment under the guidance of doctors to prepare for transplantation.
