Multiple patchy honeycomb-like changes in both lungs is an end-stage manifestation of a variety of advanced lung diseases, which can usually only be treated symptomatically, without achieving a cure. The main manifestation of multiple patchy honeycomb-like changes in both lungs on chest CT is a cluster of cystic air spaces, mostly located in the subpleura. This change is commonly seen in idiopathic pulmonary fibrosis, fibrotic nonspecific interstitial pneumonia, and hypersensitivity pneumonitis connective tissue disease-associated interstitial pneumonia.
