What is indeterminate presentation syndrome

Unidentified clinical syndrome (UCS) is a person with multiple complaints of generalized lethargy, fatigue, headache, palpitations, sleep disorders, gastrointestinal malfunction, etc. without corresponding pathology. Pathogenesis Psychosomatic factors such as overload of life events and dysregulation of the body to environmental conditions; endocrine dysfunction and ovarian hypofunction, post-ovarian resection, etc., can be causative factors. These causes can lead to various clinical manifestations of patients with autonomic dysfunction, especially increased sympathetic nervousness. Clinical classification (1) Neurotic type: Patients with predominantly psychological factors, no symptoms of autonomic dysfunction, acetylcholine test type N. (2) Psychosomatic type: The patient has 2 factors: psychological and autonomic dysfunction. Acetylcholine test S-P type. (3) Primary autonomic dysfunction type: Patients in this category do not have psychological factors, but only symptoms of autonomic dysfunction. Acetylcholine test S-P type. [1]? Clinical presentation The indeterminate presentation syndrome has a high incidence in young women and menopausal women. Patients often have multiple complaints and are more widespread, with complaints of generalized lethargy, fatigue, dizziness, headache, chest and back pain, low back pain, palpitations and shortness of breath, weakness of the lower extremities, numbness in different parts of the body, loss of appetite, and so on. If patients’ complaints are classified by system, according to statistics, systemic symptoms account for 18.8%~24.3%, neuromuscular symptoms account for 36.3%~42.1%, cardiovascular system symptoms account for 15.9%~22%, and gastrointestinal symptoms account for 20.6%~22.9%. In addition to the appeal symptoms, patients mostly complained of unbearable coldness of the hands and feet, fear of cold, and preference for warmth. Another characteristic is the multiple symptoms and temporal and spatial irregularity of patients’ complaints. Physical examination has no positive physical symptoms corresponding to the chief complaint symptoms. Diagnostic basis 1. There are many complaints and they show temporal and spatial irregularity. In addition to the spontaneous chills, a positive Shellong rise test is of clinical value to confirm the diagnosis of the disease, which is performed by measuring systolic blood pressure, diastolic blood pressure, heart rate, expired volume per waveform, cardiac index and peripheral circulatory impedance for 3 min after lying down for 15-30 min, and then taking the above-mentioned measurements after 10 min in a mobile standing position. If there is no significant change in diastolic blood pressure and heart rate, but the other indexes are significantly decreased, the test is positive. If the test is positive, it is diagnosed as positive chills and indeterminate complaints of syndromes. 2, systemic discomfort is not fixed, multiple complaints. The complaints do not match with the signs, and there are no positive findings in various auxiliary examinations. 3. Most often occurs in menopausal or young women. Psychological, social and endocrine factors are often present. Treatment The treatment principle of indeterminate complaints syndrome should be based on comprehensive treatment. It has been clinically reported that treatment can be carried out according to three types of indeterminate presentation syndrome, i.e. neuroleptic type given neuroleptic drugs and combined with psychotherapy (psychological) treatment. For the primary autonomic dysfunction type, autonomic tranquilizers combined with stellate ganglion block are used. For the psychosomatic type, psychotropic drugs, autonomic neuroleptic drugs, stellate nerve block and combined with psychotherapy and Ekuan injection therapy are used for comprehensive treatment. (1) General treatment: Patients are required to lead a regular life and participate in cultural and sports activities and labor appropriately. The patient should gradually adapt to the unpleasant environment or avoid living and working in the unpleasant environment, and maintain a good mental state. (2) Drug therapy: Methotrexate (tofisopam) is a weak tranquilizer, which has a good effect on regulating autonomic function; SF-G tablet contains vitamin B1, B6, B12 and glutamate, which has a good effect on all patients with neuralgia symptoms; NF-YI capsule contains vitamin E and glutamate, which has a good effect on menopausal autonomic dysfunction and peripheral blood circulation disorder. It is effective for menopausal autonomic dysfunction and impaired peripheral blood circulation. Chinese herbal medicine can be used to improve symptoms such as chilliness by using Angelica Sinensis Four Reverse plus Wu Yu Ginger Soup. (3) Stellate ganglion block: Stellate ganglion block is one of the most effective methods for treating indeterminate cliché syndrome. Treatment mechanism ①Block the vicious cycle of sympathetic tension. ②Improve blood circulation in the nerve distribution area, such as 75% increase in blood flow in the common carotid artery, improve cerebral blood flow, improve shoulder blood circulation, and its effect can spread throughout the sympathetic nervous system; ③Increase the patient’s defense function and have anti-inflammatory effect; ④Stabilize the function of the autonomic nervous system, vertigo and palpitations disappear after the block, blood pressure is stable, eliminate (5) The regularity of menstruation is restored, and it has therapeutic effects on habitual abortion and infertility; (6) After stellate ganglion block, the vertigo, pallor and panic when standing up no longer occur. (4) Zhi-Chuan injection therapy is performed at the site where the patient complains of pain, and Zhi-Chuan is injected into the corresponding spinal nerve innervated muscles once or twice a week for a course of 4 weeks. (5) Transcutaneous electrical stimulation: Transcutaneous electrical stimulation is the electrical stimulation of the nervous system at all levels, which can produce analgesic effects through the interaction mechanism of the endogenous neuromodulatory system. The therapeutic effect mainly relies on activation of the nervous system regulatory mechanism. This treatment method is safe, without side effects, and can be widely used. (6) Psychotherapy: Psychotherapy is also called psychiatric treatment. It is the use of psychological methods, mainly through the language and actions of medical personnel to improve the patient’s mood, raise the patient’s awareness of the disease, relieve concerns, and enhance confidence and ability to overcome the disease, in order to reduce and accelerate the treatment of the disease. (7) Acupuncture treatment method: Take the acupuncture points Foot San Li, San Yin Jiao, He Gu, Qu Chi, etc.
