In the advanced stage of femoral head necrosis, cartilage is involved, osteoarthritis occurs, and blood flow in the femoral head shows arterial occlusion. The femoral head is severely ischemic, and there may be extensive necrosis, severe loss of joint function or moderate pain, and conservative treatment may no longer be able to control the condition. At this point, if severe loss of hip function or pain occurs, artificial joint replacement should be chosen. If the symptoms are mild and the age is young (under 55 years old), the option of preserving the joint surgery can be selected, with reference to the hemodynamic performance using vascularized autologous bone graft (such as vascularized tibial greater trochanteric bone flap combined with iliac bone graft, etc.) combined with the support material surgical treatment.
