Some patients have a history of low back pain, or CT scan has been diagnosed as lumbar disc bulge or herniation, and the hip pain does not ease after years of treatment according to lumbar disc disease. In this case, there should be a high suspicion of hip joint problems, most commonly hip dysplasia. Since both lower limbs are equal in length, it is difficult to detect abnormal gait when walking, so almost all patients cannot be detected early and miss the best time for treatment. 1. What is hip dysplasia. It should be said that this is a congenital hip dysplasia, which is about five times more common in women than in men, and the exact cause is unclear. On the positive X-ray of the hip joint, it mainly shows that the acetabulum is shallow and cannot completely cover the femoral head, about 1/3 of the femoral head is on the outer side of the acetabulum, and in heavy cases, there may be a manifestation of hip subluxation. Due to the problem of joint alignment, the femoral head cannot bear the weight of the body evenly, and the medial part of the femoral head is overloaded, which increases the wear and tear of the articular cartilage, thins the cartilage, and decreases the pressure. On examination, there is pressure pain in the inguinal region, or pressure pain in the greater trochanter, and a positive “4” test with limited internal rotation of the hip joint. 2, how should be treated As the best time for treatment is missed, hip preservation treatment should be considered first at this age. Although some patients choose surgical treatment such as acetabular osteotomy to strengthen the femoral head inclusion, the long-term effect is not ideal and is not recommended. The treatment process of hip preservation: ① Weight gain should be strictly controlled, sometimes it is difficult, but it should be insisted. ② Avoid strenuous exercise and try not to climb mountains, etc. to reduce wear and tear. ③Take drugs for joint cartilage repair and protection, such as: Chinese medicine for kidney and bone, glucosamine sulfate, etc., which need to be applied for a long time. ④In case of acute pain, analgesics can be taken to relieve pain, reduce inflammatory exudation of the hip joint and reduce the internal pressure of the hip joint. 3.Treatment for women over 50 years old With the end of menstruation, the level of hormones in the body of women in this age group decreases significantly, and the quality level of bones in the whole body begins to decline rapidly. The cartilage of the acetabulum and femoral head is severely worn, and some patients are left with only the hard bone under the cartilage, and the articular cartilage is completely destroyed and absorbed. The pain in the joints is severe, making it difficult to go to the bathroom and painful at night, affecting sleep and walking with a limp, not to mention the inability to walk long distances. Some patients have difficulty standing after eating and are afraid to lift their legs, which are all manifestations of hip dysplasia complicated by osteoarthritis. On X-rays, the hip joint space is narrowed or completely disappeared, with a large number of hyperplastic bone redundancies at the edge, osteogenesis and thickening in the acetabulum, and more obvious outward migration of the femoral head. Total hip replacement can rebuild the function of hip joint, relieve pain and restore normal gait. At present, the service life of the fourth-generation total ceramic hip joint is generally more than 20 years, and the long-term effect is good, so you can consider choosing it if you have the conditions.
