Clinical experience in the treatment of acute gouty arthritis using ophthalmic acupuncture combined with giant stabbing and distant stabbing

Gouty arthritis is a disease caused by the disorder of purine metabolism and the increase of uric acid in blood, resulting in the deposition of uric acid crystals in joints and subcutaneous tissues. For the acute attack of this disease, most of the western medical treatment uses drugs, such as colchicine, non-steroidal anti-inflammatory drugs, corticosteroids, etc., which have the disadvantages of complicated medication and toxic side effects. Traditional acupuncture treatment of this disease mainly follows the principle of local meridian, mainly taking the five points of the spleen, kidney, bile, stomach and bladder meridians near the joints of the affected limbs, the original points and dorsal points, such as diaphragm Yu, bladder Yu and kidney Yu, and adding and subtracting according to the different types of evidence. Eye acupuncture is a micro-acupuncture technique performed around the orbit to treat systemic diseases, and it is a special acupuncture method of acupuncture. The theoretical basis lies in the eye and meridian communication, access to internal and external, contact with the internal organs. As early as in the Nei Jing, there is a view that “the five viscera of the eye are divided”. In the “Spiritual Pivot – Mouth Question”, “The eye is the place where the meridians are gathered”. The Spiritual Pivot – Dazed Theory also mentions that “the essence of the five viscera and six bowels are all injected into the eyes as essence”. It means that the eye relies on the moistening of the essence of the five viscera and six internal organs in order to play its physiological role. The “Spiritual Pivot – Evil Qi Viscera Disease Form” points out that: “Twelve meridians, 365 veins, their blood and qi all go up to the face and go to the orifices. The essence of Yang Qi goes up to the eyes and is the essence. This indicates that the meridians are all directly or indirectly connected to the eye through the surface and lining meridians. The unique theory of the five chakras and eight contours, created by later medical practitioners, connects all parts of the eye to the internal organs and can be used to explain physiopathological mechanisms. The lower jiao zone in eye acupuncture is used to treat diseases below the navel, including lower limb pain, as well as the upper jiao zone. It can also be used with the contralateral heart zone, which is based on the theoretical basis of the “19 Articles of the Disease Mechanism” in Su Wen – Zhi Zhen Yao Da Lun, “All pains and itches belong to the heart”. The use of the contralateral heart area is a symptomatic treatment for the acute attack of the disease, which causes severe pain and disturbance of the mind and sleep. Eye acupuncture is mainly used to treat ipsilateral diseases, and eye acupuncture meridian conduction routes each go to its ipsilateral side, so the main points should be selected from the affected side. Supporting points mainly play a role in adjusting the internal organs, take the opposite side of the point can also be. 2, giant stab method giant stab method is a left disease to take the right, right disease to take the left, left and right cross-acupuncture treatment method. Su Wen – Miao thorn theory said: “The emperor: I would like to hear Miao thorn, left to take the right, right to take the left, what? What is the difference between this and the giant stab? The man said: the evil guest in the meridian, the left side of the right side of the disease, the right side of the disease, the left side of the disease. There is also a shift, the left pain has not yet and the right vein first disease, so, must be giant prick of the, in order to its meridian, non-loop veins. If the disease of the luo is in the same place as the meridian, it is ordered to be pricked. Su Wen – regulation of the meridian: “the body form has pain, the nine wait for Mo disease, then Miao prick of the pain lies in the left, and the right pulse disease, then giant prick. The “standard peculiar fugue” also clearly pointed out that “live pain, move pain to take the intersecting veins”. Ling Shu – stabbing section of the real evil “” with needles and so on, is to regulate the qi “. Therefore, acupuncture can be used to regulate the qi flow, so that it is not painful. From the perspective of meridian physiology, the meridian system is distributed in the yin and yang of the hands and feet, is symmetrically distributed on the left and right, the eight channels of the Qi meridians are symmetrically distributed in the front and back, and the meridians themselves have to be “connected up and down, left and right through” to “intersect with the tendons” and so on. The whole body meridians are connected from left to right and up to down, and by acupuncture on the opposite side of the acupuncture points, it can smooth the qi and blood, unblock the meridians and thus cure the disease. As pointed out in “Acupuncture and Moxibustion”, “Giant acupuncture is to pierce the meridians, the pain is on the left and the right vein is sick, then giant acupuncture, left pain acupuncture right, right pain acupuncture left, in its meridian also. The mechanism of the giant prick method of healing is mainly to adjust the meridians on both sides of the body in order to make it smooth. 3.Distant stabbing method The distant stabbing method belongs to one of the nine stabs in the Nei Jing. For example, in the chapter of “Ling Shu – Official Needle”, it is said that “the distant stabbing is when the disease is on the top, take the bottom and stab the Fu Yu”. Therefore, the distant path stabbing is a kind of stabbing method in which the upper disease is taken from below and the lower disease is taken from above, following the distant path of the meridian to take the points. The Su Wen – Wu Chang Zheng Da Lun says: “If the disease is above, take it below; if the disease is below, take it above; if the disease is in the middle, take it at the side”. “When the disease is above, take the points below” refers to the head, face, and upper extremities. Specifically, head and facial disorders can be taken through the distant channels, and upper limb disorders can be taken from the lower limb with the same name. In the Nei Jing, the theory of meridians is discussed, detailing the upward and downward flow of the eponymous meridians of the hands and feet, the left and right intersection of the eponymous meridians of the two sides, and the interrelatedness of the eponymous meridians of the hands and feet in the transmission and treatment of disease. Similarly, “if the disease is in the lower part of the body, take the upper part”. In addition to taking the disease from the lower part of the body, the painful disease in the lower extremity can be treated by taking the corresponding point in the upper extremity according to the theory of “the same qi seeking”, i.e., the upward and downward penetration of the same named meridian in the hands and feet. Clinical experience in the treatment of acute gouty arthritis with ocular acupuncture combined with giant acupuncture and distant acupuncture 1. Therefore, the first and foremost task of patients coming to the clinic is analgesia. Professor Fu Wenbin uses the main acupuncture point of eye acupuncture to relieve pain, and the effect is more satisfactory. Generally, the upper or lower focus area is selected according to whether the painful joint is in the upper or lower extremity. Needle method: take 0.35 mm X25 mm disposable milli-needle and stab people quickly along 0.5 cm outside the orbital rim, the depth of straight or oblique stabbing is to reach the orbital periosteum. 2, giant stabbing method combined with distant stabbing method to further relieve pain Usually, after using eye needle therapy, the patient’s joint pain has been greatly reduced, but the joint still often remains a little tight pain and discomfort. At this time, use the giant prick method combined with the distant stab method to further relieve the patient’s joint pain. The “giant stab” is taken on the opposite side of the affected joint, and the “distant stab” is taken on the upper extremity (for the affected joint in the lower extremity), and the combination of the two is used to take the points of the second stab by crossing the points on the left and right. If the affected joint is the first metatarsophalangeal joint on the left, the right side Sanqian point is chosen; if the affected joint is the right ankle joint, the left side Yangchi or Yanggu point is chosen. In addition to the above conclusion that “lower extremity disorders can be treated with the same name as the upper extremity”, the choice of these specific points is based on the following theory: “The injection is a transfusion point”, and the Sanquan point is one of the five transfusion points of the large intestine meridian. It is like water flowing from the shallow to the deep, and the pulse is strong. It is like water flowing from shallow to deep, and has a strong pulse. It is closely related to the TCM etiology of the disease: internal stagnation of phlegm, dampness, and toxicity, and stagnation of phlegm and turbidity in the meridians. The Yangchi point is the original point of the Sanjiao meridian, “Sanjiao is the official of the ductus, and the watercourse comes out of it”, and the “Classical Classic – Tibetan Elephant” states: “When the Sanjiao Qi is cured, the vasculature is open and the watercourse is favorable”. “When the five viscera have diseases, the twelve original points should be taken”, acupuncture original points can make the original Qi of Sanjiao pass through and promote the metabolism of fluids in the body. 3. Comparison with traditional acupuncture methods for treating this disease For acute attacks of gouty arthritis, two needles are generally used for analgesia (the first needle for the eye and the second needle for the body). The acupuncture points are concise, and there is no need to use additional external treatments such as moxibustion, blood pricking, fire acupuncture, and herbal compresses. After reviewing the literature on acupuncture treatment for this disease in the past 10 years, it was found that most physicians use acupuncture points such as the well point, the healing point, the losing point, or the aye-point as the main acupuncture point, and then add local acupuncture points on top of the main acupuncture point according to the type of evidence or the location of the pain, and take more acupuncture points. For example, Kuang Wu treated 70 patients with this disease with simple acupuncture, taking the well point of the meridian where the lesion is located in the acute stage. In the acute stage, the well points of the meridian where the lesion is located are used. In remission, take the affected side of the foot Sanli, Yanglingquan, Yinlingquan, Sanyinjiao, Tai Bai, and Bafeng; on the affected side of the upper limb, take Quchi and Baxi. Ma Xiaoping. The main acupuncture points are Quchi, Blood Sea, Sanyinjiao, and Taixi, with addition and reduction according to the symptoms: shoulder pain with shoulder Calvary and shoulder well; elbow pain with Yangchi and Waiguan; knee pain with knee eye and Yanglingquan; ankle pain with Qiuhui and Hanging Bell; bunion pain with Gadu and Taichong. Xuan Lihua and other acupuncture treatment of gouty arthritis 38 cases, the choice of Taichong, Yanglingquan to clear heat and relieve pain, foot Sanli, Sanyinjiao to strengthen the spleen and help transport, Zhubin to supplement the kidney to clear turbidity, branch communication to benefit the three jiao. 3.Example of case Huang, female, 67 years old, first diagnosis of El period: July 8, 2008, last follow-up date: September 2, 2010. The patient had a history of recurrent gout attacks for 4 years and insisted on going to the clinic once a week for the past 2 years for acupuncture treatment to control her condition; on September 1, 2010, after eating spinach for dinner, she suddenly woke up at night with severe pain in her right ankle without taking any medication. He went to the outpatient clinic the next morning. Physical examination: localized redness and swelling of the inner and outer ankle joints of the right foot, skin temperature is slightly high, pressure pain is obvious, movement is limited, and walking is difficult. The patient was asked to self-evaluate the pain level by visual analogue scoring (VAS): the patient scored 10 for self-perceived pain. Western medicine diagnosis: gouty arthritis (acute stage); Chinese medicine diagnosis: gout (damp-heat nodules). Treatment: The right hypochondriac area was selected, the needle handle was lightly scraped, and the patient was told to move the right ankle joint slowly when he complained of soreness and swelling at the needle prick. After about half a minute, the patient felt that the pain was greatly relieved and scored 3 on the visual analogue score (VAS), but still felt tightness and discomfort in the outer ankle. The patient’s remaining tightness and discomfort were further relieved. After 2 min, the patient complained that the pain had basically disappeared and the VAS score was 0.5 on the third measurement. In the treatment of acute gouty arthritis patients, Fu Guiding did not stick to the identification of internal organs and add corresponding acupoints, but started from the key point of pain, which is the key to pain, and applied the ideas of overall identification and meridian identification, combined with the classics, to refine the appropriate acupuncture point method, so that patients can largely avoid secondary pain caused by taking more acupuncture points. The clinical need for rapid and precise therapeutic effect is worthy of reference and promotion.
