If the unstable displaced fracture is not too old and in good health, tibial tuberosity bone traction can be used for 8-10 weeks with manipulation to reset the fracture. Elderly patients who are not able to stay in bed for a long time should be treated with a closed-brace external fixation brace, or closed-displacement internal fixation. Severe comminuted fracture, or too old to accept bone traction and surgery, can consider skin traction treatment, try to let the patient to take semi-recumbent position, do not affect the patient’s life. The majority of trochanteric fractures heal successfully, and non-union rarely occurs. Elderly and long-term bed-ridden patients are prone to pneumonia, so they can use sling retraction training and active sputum evacuation to prevent complications such as drop pneumonia, pressure sores and urinary tract infections. During the period of immobilization, it is necessary to actively exercise the quadriceps muscle and ankle flexion and extension activities, traction immobilizers can take a sitting position in 2-3 weeks, and practice hip lifting activities, 3-4 weeks later, pull the halter with both hands, and step on the bed with the healthy foot to do hip lifting activities, and the buttocks can leave the bed surface completely to practice hip and knee joint activities. After 6-8 weeks, when removing the traction and walking on the ground with crutches, it should be determined according to the fracture healing condition shown in the X-ray, and the crutches can be removed only after complete stabilization.
