Early symptoms of gastric cancer are atypical. Early manifestation of epigastric discomfort is the most common initial symptom in gastric cancer, about 80% of patients have this manifestation, which is similar to indigestion. If abdominal pain occurs, it is generally light and irregular, and cannot be relieved after eating. These symptoms are often not taken seriously by patients and are easily mistaken for gastritis or ulcer disease when seeking medical attention. Therefore, middle-aged patients should be given further examination if they have the following conditions to avoid missing the diagnosis: 1. no previous history of gastric disease, but recently developed unexplained epigastric discomfort or pain, which is ineffective after treatment; 2. previous history of gastric ulcer, and recently the regularity of epigastric pain has changed, and the degree is getting worse. If the symptoms are relieved, but there is another occurrence in a short period of time, the possibility of gastric cancer should also be considered and further examination should be conducted in time. Gastroscopy should be performed to clarify the diagnosis, and biopsy pathological examination can be done if necessary. Common symptoms include: 1. Upper abdominal pain: about 1/4 patients have the same pain pattern as peptic ulcer, while elderly people have dull pain sensation, mostly manifested as abdominal distension, and upper abdominal pain accounts for 30%-70%. If the above abdominal pain often recurs, or the interval is getting shorter and shorter, it should be alarmed. 2.Sense of fullness in upper abdomen: it is often the earliest symptom of gastric cancer in the elderly, sometimes accompanied by belching, acidity, nausea and vomiting. If the cancer is located at the entrance of the stomach (cardia), the patient may feel that the food is not smooth; if the cancer is located at the exit of the stomach (pylorus), the patient may vomit the rotten overnight food when there is obstruction. 3.Loss of appetite, wasting and weakness: According to statistics, about 50% of elderly patients have obvious loss of appetite, increasing wasting and weakness, and 40%-60% of patients seek medical treatment for wasting. If accompanied by epigastric pain, and can exclude hepatitis, should be more alarmed. 4. Gastrointestinal bleeding: vomiting blood, black stool and positive fecal occult blood are common. Small cell lung cancer chemotherapy regimen can show signs of small amount of bleeding in the early stage of gastric cancer. The percentage of vomiting blood is 7%-15%, and the percentage of black stool is 23%-45%. The rate of positive fecal occult blood is higher, 87.6% for gastric body cancer and 64.4% for cardia cancer. If the fecal occult blood is consistently positive, it is helpful for the diagnosis of gastric cancer.
