Stomach cancer is a common malignant tumor. The population-adjusted mortality rate of gastric cancer in China (male: 40.8/100,000, female: 18.6/100,000) is 4.2-7.9 times and 3.8-8.0 times higher than that of developed countries in Europe and the United States, respectively. Although its incidence and mortality rates have decreased in China’s large cities over the past half century, according to the latest statistics (2002-2004 data), the crude mortality rate of gastric cancer in China’s large cities such as Shanghai is 52.24/100,000 for men, which is still the second highest incidence rate of malignant tumors, and 29.26/100,000 for women, which is the third highest incidence rate of malignant tumors. In the past 10 years, chemotherapy, as the main means of adjuvant treatment for gastric cancer, has improved in efficiency as new anti-cancer drugs have been put into clinical application, but there is still no breakthrough in the treatment of gastric cancer. Chinese medicine has great advantages in anti-metastatic recurrence, prolonging survival and improving life quality, and occupies an important position in the treatment of gastric cancer. Chinese medicine is an important part of comprehensive tumor treatment, and with the development of modernization of Chinese medicine, Chinese medicine treatment methods have been enriched. In the whole process of stomach cancer treatment, we should always take Chinese medicine theory as the guide, discriminative treatment as the core, combine discriminative evidence with disease, reasonably use various Chinese medicine treatment methods, and maximize the advantages of overall Chinese medicine treatment. Patients should take oral tonics from the time of surgery. At this time, due to the long fasting time, the nutritional status and immune function of patients are damaged, and the postoperative trauma has suppressed the gastrointestinal movement and digestive and absorption functions, so TCM practitioners often treat the patient with the method of nourishing Yin and nourishing Blood, strengthening the spleen and benefiting Qi, such as Si Jun Zi Tang, Ba Zhen Tang, Si Mo Tang, etc., in order to rapidly improve the physical and general condition of patients. Evidence-based medical evidence shows that postoperative radiotherapy and chemotherapy are important means to prevent recurrence of gastric cancer, and postoperative radiotherapy can increase the 3-year recurrence-free survival rate from 32% to 48% for patients with T3, T4 and lymph node positive gastric cancer. 9.2~13.9 months through active treatment. However, radiotherapy and chemotherapy have their own insurmountable toxic side effects. While killing tumor cells, radiotherapy and chemotherapy also damage the normal tissues and immune function of human body, so we should focus on supporting the root, which can not only reduce the adverse effects caused by radiotherapy drugs, but also improve the immune function of the body to help complete radiotherapy and chemotherapy on schedule. At the same time of radiotherapy, oral tonics to strengthen the spleen and stomach can be taken to reduce the adverse effects of nausea and vomiting; if bone marrow suppression occurs, tonics to strengthen the spleen and kidney or Chinese medicine should be given to help restore the hematopoietic function. After radiotherapy, in addition to regular review, the treatment should also be combined with evidence-based treatment. However, when applying anti-tumor herbal medicines for clearing heat and detoxifying, resolving phlegm and dispersing nodules, attention should be paid to regular replacement to avoid hepatic and renal toxicity, and a certain drug should preferably not be used for more than 3 months. In addition to oral tonics, Chinese patent medicines with anti-cancer effect can be added, such as Huachansuo tablets, demethylzebutin tablets, Antico capsules, compound zebutin capsules, etc. to strengthen anti-metastasis and anti-relapse ability, but it is recommended to use only one kind of adult medicine in the same period and switch to another one in half a year, so as to control the treatment cost and prevent the adverse effects brought by long-term application. For advanced gastric cancer, if conditions allow, it is also possible to combine TCM comprehensive treatment on the basis of Western medicine treatment, and add Chinese patent medicines while taking discriminative treatment tonics, and patients with conditions can be hospitalized 4-6 times a year for about 20 days each time, with 1-2 intravenous Chinese medicine preparations, such as Huachansu injection, Adi injection, compound bitter ginseng injection, anti-cancer flat injection, etc., together with Chinese medicine food therapy, sound therapy, foot bath, acupuncture, external application or enema and other comprehensive treatment methods. Adhering to the principle of discriminative treatment Discriminative treatment is the essence of Chinese medicine and can best reflect the academic thinking of Chinese medicine. For the development of gastric cancer, TCM believes that it is mostly due to congenital deficiency, weakness of righteousness, invasion of external evil, or disorderly diet, disorder of emotion and disorderly labor, etc., resulting in loss of spleen health, loss of stomach harmony and descent, gathering dampness and phlegm, blocking blood vessels with phlegm, poor blood flow, stasis and toxicity, which are blocked in the stomach and epigastrium. The accumulation of phlegm will further lead to dysfunction of the internal organs of the body, and eventually make the stagnant blood, phlegm and evil toxins fight with each other and develop. And the positive energy will be further injured, forming a vicious circle and accelerating the development of the disease. In short, the ultimate pathogenesis of gastric cancer is deficiency of positive energy, phlegm-dampness coalescence, stagnation of blood and heat toxicity. It can be seen that gastric cancer also belongs to the category of spleen and stomach diseases in Chinese medicine, which must adhere to the principle of differentiation and treatment, and make comprehensive use of methods such as regulating qi and strengthening spleen, removing dampness and phlegm, and activating blood stasis. Generally speaking, gastric cancer is divided into six types: liver-stomach disharmony, liver-spleen disharmony, spleen-stomach cold deficiency, phlegm-dampness coagulation, internal stasis and toxin obstruction, and qi-blood deficiency, etc. According to different types, corresponding treatment methods such as soothing the liver and stomach, strengthening the spleen and softening the liver, warming the middle and dispersing cold, drying dampness and resolving phlegm, activating blood circulation and eliminating stasis, and nourishing qi and blood are often given. Therefore, while taking care of the function of the spleen and stomach, eliminating the evil (anti-tumor) is another major problem we face. Anti-cancer herbs are most suitable for those who have fever, thirst, constipation, burning and painful lumps, and red tongue with yellow coating and number of veins. Herbs such as bitter ginseng, white flowered snake’s tongue, lobelia, dandelion, white ying, verbena, half branch lotus, vine root, snake berry, etc. have good anti-tumor effects. On the contrary, for those who have cold fear, loose stools, white tongue with light coating or teeth marks on the side, and sunken and tight pulse, bitter-cold products should be used with caution, and anti-cancer drugs can be chosen from Zanthoxylum, Toadstool and Dried Toad Skin. If the patient has stabbing pain, dark tongue, thin pulse and other manifestations of blood stasis, anti-drugs such as whole scorpion, leech, salvia, curcuma, wuling fat and penetrating nail can be selected. If there are signs of internal obstruction of phlegm and dampness, such as stomach and epigastric fullness, thick tongue coating, etc., then Chinese herbs such as coix seed, pig ling, calamus, etc. should be chosen to have dual functions of dampness and anti-cancer. In conclusion, although the treatment of gastric cancer emphasizes the elimination of evil and anti-tumor, it should be combined with dialectical evidence, and anti-cancer herbal medicines are often used on the basis of the diagnosis and treatment, and should not be used blindly. It should be emphasized that for patients with unresectable gastric cancer, in the early stage, the treatment should be based on clearing heat and detoxifying, softening hard nodes, removing phlegm and eliminating stasis, etc., while in the middle stage, the treatment should be based on both attacking and supplementing, and in the late stage, the treatment should be based on supporting the righteousness. In the late stage of gastric cancer, because of the weakness of the positive energy, it is forbidden to use drugs with great toxicity and bitter cold to hurt the stomach. It is also pointed out that the treatment of accumulation should not be rushed, but should be “repeatedly attacked and tonicized, in order to calm the stomach”. It can be used as a reference. From a dialectical point of view, the human body and disease are a unity of contradictions, that is, the cause of disease is attached to the human body and the human body fights against it because of disease. In this contradictory unity, the focus and core should be the “human being”. Therefore, the treatment must be people-oriented and fully mobilize the positive energy of the human body. As stated in Su Wen? As stated in “Shanggu Tianzhen Lun”, “When the righteousness exists within, the evil cannot be interfered with. To summarize, positive energy is all the power that can resist evil, including both mental and physical aspects. As a clinician, his duty is to mobilize all the positive energy of the human body to fight against gastric cancer, so as to ultimately achieve the goals of survival with tumor, improving quality of life, and increasing long-term survival rate. In addition to paying attention to the tumor, the clinician should also pay attention to the changes of the patient’s subtle symptoms, such as the patient’s poor sleep may affect the physical recovery, and the irritable sweating may affect the patient’s mood, etc., which should be paid attention to and actively treated. At the same time, we should also pay attention to and guide the patient’s diet, encourage the patient to establish confidence in overcoming the disease, eliminate the patient’s fear of the disease, and ask the patient to maintain a good state of mind. Li Dongyuan pointed out in his treatise on the spleen and stomach that “internal injury to the spleen and stomach causes all kinds of diseases.” This indicates the important role of the spleen and stomach in the pathogenesis of the body. The spleen and the stomach are the source of the posterior, the source of Qi and blood biochemistry. As the old saying goes, “Those who get water and grain will prosper, those who lose water and grain will die”, and “once the stomach qi is defeated, all medicines are difficult to cure”. Therefore, in the treatment of gastric cancer, we must always keep in mind to protect the spleen and stomach. Specifically, we should pay attention to four points: First, we should be careful of thick and greasy tonic that hinders the stomach. Patients with gastric cancer, especially in the late stage, often show different degrees of deficiency of yin and yang qi and blood, so if we put in a lot of big tonic products, it may hinder the stomach qi, which is counterproductive. In the treatment, no matter attacking the evil or helping the positive, we should not forget to combine appropriate amount of qi and stomach products, such as Chen Pi, Citrus aurantium, Buddha’s hand, Mu Xiang, Sha Ren, and Han Xia. Third, the use of drugs to do less and fine, as far as possible, choose a single taste and effect, dual-competent drugs, in order to achieve less taste, light dose, the stomach and internal organs to accommodate and not lose its effectiveness of the purpose. On the contrary, the medicine is mixed, the stomach and the internal organs are not resistant, there is no effect to speak of. Fourthly, the patient should be reminded to pay attention to the protection of spleen and stomach functions in daily life, avoid overexertion, exercise in moderation, and live a regular life. Patients should be instructed to eat light, fresh and easily digestible food, and avoid spicy and stimulating, hard and rough, greasy food. Conclusion Gastric cancer is a difficult disease. Long-term clinical practice proves that the treatment idea of combining evidence and disease on the basis of evidence-based treatment can effectively prolong the survival period and improve the survival quality of patients. Chinese medicine will definitely play a greater role in the prevention and treatment of gastric cancer.
