Acute myocardial infarction (AMI), also known as acute myocardial infarction, is the acute necrosis of part of the myocardium due to persistent and severe myocardial ischemia. Common causes include coronary artery lumen thrombosis, coronary artery spasm, atherosclerotic plaque rupture or sub-plaque hemorrhage. This disease is a sudden and dangerous emergency, and some patients may have aura symptoms before the onset. Some patients may have aura symptoms before the onset of the disease, such as dullness and dull pain in the precordial region, pain sometimes radiating to the arm or neck, accompanied by nausea, vomiting and cold sweat. In this case, stop any heavy physical activity immediately, avoid emotional excitement to reduce myocardial oxygen consumption, and take rapid-acting vasodilator drugs such as nitroglycerin tablets or isoamyl nitrite orally, some patients can avoid myocardial infarction. Since cold stimulation can lead to coronary artery spasm and induce myocardial infarction, patients with coronary heart disease should pay attention to keeping warm when going out in the cold season, adjust the time of going out for exercise and avoid prolonged contact with cold water to prevent the occurrence of myocardial infarction. When acute myocardial infarction occurs, patients feel severe and persistent pain under the sternum or precordial area, accompanied by pale face, panic, shortness of breath and cold sweat, etc. Some elderly or diabetic patients do not feel severe chest pain, or due to the ischemia of the lower wall of the myocardium as a sudden onset of severe pain in the upper abdomen, but other symptoms will be more severe, rest and taking fast-acting vasodilator drugs can not relieve the pain. Once the above symptoms occur, you should immediately dial the 120 emergency number or a nearby hospital. Before help arrives, you can take a deep breath and then cough hard, which produces chest pressure and vibration, the same effect as the chest cardiac massage in CPR, when coughing hard can win time for subsequent treatment and is an effective self-help method. According to medical statistics, the first few hours of myocardial infarction are the most dangerous period, and about 2/3 of patients die before seeking medical attention. Moving the patient in a panic, carrying or helping the patient to walk to the hospital can increase the burden on the heart and expand the scope of myocardial infarction, even leading to the patient’s death. Therefore, it is very important to keep the patient calm during first aid, and family members or rescuers should not panic, but should resuscitate the patient in place, let the patient slowly lie down and rest, and minimize unnecessary changes in their position. For patients who are extremely nervous, if the blood pressure is normal or high, 10 mg of Valium can be given orally, but for those with low blood pressure after the onset, Valium is prohibited to avoid aggravating shock. During the waiting period, if the patient appears pale, wet and cold hands and feet, rapid heartbeat, etc., it mostly indicates that shock has occurred, at this time, the patient can be made to lie down, feet slightly padded, remove the pillow to improve the cerebral ischemia. If the patient suddenly falls into a coma or convulsion, suggesting that the heart suddenly stops beating, the family should not pick it up and shake it to call, but should immediately use the first aid measure of boxing the precordial area to make it beat again. Cardiopulmonary resuscitation is an important measure to establish effective blood circulation to ensure the perfusion of the brain and other important organs and save the life of the patient, the earlier the implementation of the patient in respiratory and cardiac arrest arrest, the higher the success rate of resuscitation. However, I found in the clinical emergency, because China’s emergency network is still to be improved, coupled with road congestion and other reasons, 120 from the reception to the scene, often need a process, so that many patients lost the best rescue time. Therefore, I hereby call for more investment in first aid from the government’s point of view, improve the first aid network and gradually develop to the three-dimensional space of land, sea and air, so that more critical patients can be rescued in time. It is also suggested to set up CPR courses in junior high schools or high schools, and establish CPR training places in communities to raise the awareness of resuscitation for all people.
