Precautions before treatment of tubulointerstitial nephritis uveitis syndrome: The treatment of tubulointerstitial nephritis is mainly systemic application of glucocorticoids, the commonly used preparation is prednisone, oral dose is generally 1m/(kg-d), the treatment time varies from 3 weeks to 12 months, the treatment time should not be too short, should be in 5 to 12 months as appropriate, rapid reduction of drugs is likely to cause an increase in serum creatinine levels. Treatment of uveitis is mainly with glucocorticoids and ciliary muscle paralyzing agent drops for eye spotting. For bilateral acute and severe anterior uveitis, dexamethasone eye drops should be given once an hour and then gradually decreased in frequency depending on the control of inflammation. If eye drops dabbing treatment causes an increase in IOP, adjust the frequency of dabbing or use glucocorticoids with few side effects, such as flumethoxylated prednisolone and rimexolone, and give anti-glaucoma drugs dabbing treatment. Ciliary muscle paralyzing agent is usually chosen as 2% postmastropin ophthalmic ointment, 1 time/d. Later, as the inflammation decreases, the frequency of eye spotting is gradually reduced. Non-steroidal anti-inflammatory drops can also be used for eye spotting treatment. If the treatment effect is not satisfactory, oral glucocorticoid treatment can be considered, the general dose is 30-40mg/d (prednisone), morning dose, about 1 week after the dose can be reduced, the treatment time is generally not too long, the treatment should pay attention to the side effects of this drug. If the patient is not sensitive to glucocorticoids (this is mostly in patients with posterior uveitis), other immunosuppressive drugs can be considered, such as nitrogen mustard phenylbutyrate 0.05-0.1mg/(kg-d), methotrexate (7.5-15mg/week), cyclosporine 3-5mg(kg-d), azathioprine 1-2mg(kg-d), etc. These drugs can be used alone or in combination. However, liver and kidney function and/or blood tests should be performed regularly during the treatment to avoid serious toxic side effects. TCM evidence-based treatment is helpful in reducing renal impairment, promoting the recovery of renal function and uveitis, and reducing the toxic side effects of immunosuppressive drugs.
