Introduction to Interventional Ultrasound and What to Know

Interventional ultrasound refers to various aspirations, injections, puncture biopsies, tube drainage, tumor ablation and other operations done under the surveillance or guidance of real-time ultrasound, which are minimally invasive, effective, widely indicated, relatively easy to operate, relatively inexpensive and radiation-free. Interventional operations can be done on an outpatient basis and can achieve comparable effects with surgical procedures while avoiding certain surgical procedures. Currently, ultrasound-guided interventions are receiving more and more clinical attention and have become a routine treatment method. 1. Ultrasound-guided aspiration (diagnostic aspiration and therapeutic aspiration): indications: pleural fluid, ascites, abscess, hematoma, cyst, polycystic liver, polycystic kidney, bile leak, etc.; contraindications: very small amount of effusion; severe bleeding tendency, coagulation dysfunction; mental disorders that do not cooperate. 2.Ultrasound-guided cyst puncture sclerotherapy: Indications: liver cysts and kidney cysts with diameters greater than 5 cm; breast cysts, thyroid cysts, etc.; Contraindications: severe bleeding tendency; alcohol allergy; encapsulated cysts; cysts of renal pelvis origin; liver cysts connected with the biliary tract. 3.Ultrasound-guided aspiration biopsy: Indications: diffuse abdominal parenchymal organ lesions requiring pathological diagnosis; solid masses of various organs with unknown clinical diagnosis requiring pathological diagnosis; tumor chemotherapy requiring pathological diagnosis and immunohistochemistry to guide the selection of chemotherapy regimen before chemotherapy; lymph nodes, pleura, soft tissues, gastrointestinal tract, etc.; Contraindications: coagulation disorders, severe bleeding tendency Those who do not cooperate with mental disorders; those whose lesions are too small to be observed; huge hemangiomas or encapsulated lesions. 4, ultrasound-guided drainage: indications: abdominal abscess, hematoma; obstructive jaundice; hydronephrosis; pancreatic pseudocyst; bile leak; pleural fluid; ascites, etc. Contraindications: coagulation dysfunction, severe bleeding tendency; massive ascites; liver and kidney failure; bile duct and renal pelvis dilation is not obvious. 5.Ultrasound-guided tumor ablation therapy: Indications: local treatment of malignant tumors of liver, kidney and other organs; pain relief for patients with advanced cancer; those who are unwilling to undergo surgical treatment; those who recur or progress after surgery or radiotherapy; liver transplantation bridge. Indications for radical treatment: primary or secondary tumors of liver and kidney; maximum tumor diameter ≤ 5 cm; number less than 5; liver function Child classification A or B; safe distance from heart, gallbladder, gastrointestinal and hepatic hilum; no cancer embolism and distant metastasis. Contraindications: severe coagulation dysfunction; Child C grade of liver function. Precautions before interventional ultrasound treatment 1. If your condition requires ultrasound interventional guidance and further diagnosis and treatment, please visit the relevant specialist outpatient clinic or ultrasound diagnosis and interventional treatment specialist first, and let the doctor understand your condition and assess whether you need a series of examinations and operations of interventional ultrasound, then the doctor will issue an application form for interventional ultrasound, inform you of the operation process, obtain your consent, and improve the relevant preoperative tests. The procedure will be carried out after the procedure is completed. 2. If you want to perform ultrasound-guided interventional operations on the liver, gallbladder and bile ducts, pancreas and spleen, please fast and drink for at least 8 hours. If you want to perform ultrasound-guided thoracic or abdominal aspiration, kidney, thyroid, breast, body surface mass, lymph node intervention, please eat properly. 3.If you have had interventional ultrasound treatment in our department, you can directly register for the outpatient number of “Ultrasound Diagnosis and Interventional Treatment” and then come to our department for follow-up consultation. 4.Pre-operative tests include blood routine, coagulation function, HIV, syphilis test, if you have serious heart disease or other systemic diseases, the test should be conducted according to the specific situation.
