A revolution in spine care

(Chiropractic Branch of the Chinese Society of Traditional Chinese Medicine) On July 29, 2015, << span=""><< span="">The Occupational Dictionary of the People’s Republic of China>> was released, and Chinese chiropractors were included in the Chinese medicine profession, which sounded the horn of the revolution of spinal diagnosis and treatment, and ushered in the dawn of the era of “vertebral curvature theory”! In this paper, we will briefly summarize and discuss the course of Professor Wei’s creation of the theory of vertebral curvature from the formation of the background and development of a new era of spinal diagnosis and treatment, as well as the great influence on modern spinal medical diagnosis and treatment, and give a glimpse to the readers. I. The background of the time when Wei Yizong created the theory of vertebral curvature Since 1934, Mixter and Barr of Harvard Medical School in the United States co-authored the “rupture of the intervertebral disc involving the spinal canal” published in the New England Journal of Medicine, 19 patients with low back pain attributable to hernia of the nucleus pulposus, and surgically proven and named After the rupture of the intervertebral disc, it has been hailed as the inauguration of the so-called “era of the disc (dynasty of the disc)”, and for more than half a century, herniated discs have caused low back pain to become a “golden rule” like the “golden rule” of low back pain. Golden Rule” like etiology. The ensuing surgical resection therapy has become popular around the world. Overseas experts have recognized the shortcomings of disc surgery, and according to the American Academy of Orthopaedic Surgeons in 2001, the success rates were 45% and 54%, of which 35% required reoperation. In China, this treatment has been popularized to county hospitals. According to some data reports, the rate of surgery for herniated discs in China has reached more than 80%, resulting in a “three-level insanity” and even “indiscriminate killing of innocents” (Shi Qi’s words). In addition, there are people blindly to deal with the intervertebral disc, the introduction of some foreigners have been abandoned by the foreign “foreign therapy”, bragging about its efficacy, to deceive the sick and wounded, such as: what nucleolytic surgery, cut suction, radiofrequency ablation, removal and so on. But the reality tells us that after intervertebral disc surgery, due to disc removal or ablation, the intervertebral space narrows, vertebral body collapse; on the other hand, disc herniation caused by structural mechanical disorders, such as lumbar scoliosis, vertebral curvature straightening failed to rectify the secondary herniation of multiple discs, degeneration; surgical trauma and bleeding caused by the spinal canal scar tissue hyperplasia and adhesion; surgical disruption of the spinal column’s stability, caused by spinal slippage. Surgery disrupts the biomechanics of the spine, thus secondary to traumatic bone and fibrous structure hyperplasia; posterior soft tissue protrudes into the spinal canal and adheres to the dura mater after total laminectomy or hemilaminectomy; thickening of the lamina caused by spinal fusion; surgical inadvertence, the spinal canal is left with crushed bone fragments. This series of surgical complications and sequelae have been constantly appearing in newspaper reports and papers in academic journals. In real life, some doctors are driven by economic interests, actively mobilizing patients to do surgery, disc surgery, the surgeon will tell you: “your discectomy is very successful, the following recovery or not is your own business.” There are also some people who boast of ancestral panacea to keep patients in bed, and preach that disc shrinkage or recovery is their own credit to hoodwink patients, not realizing that the herniated disc itself has the function of absorption and reset. Undoubtedly, the intervertebral disc doctrine has relieved pain for many injured and sick patients, but it has led to many complications and sequelae of the most serious lack of, as pointed out by Jamcs.H. Beaty, president of the American Academy of Orthopaedic Surgeons, “Removal of the intervertebral disc is to relieve sciatica, but it does not restore the lumbar spine’s normal mechanics of the problem.” Second, Wei Yizong pioneered the foundation of spinal diagnosis and treatment of vertebral curvature theory Prof. Wei Yizong has been practicing medicine for fifty years, and in the 1970s, after studying orthopaedics at Guangxi Medical University, he carried out intervertebral disc surgery and familiarized himself with academician Ge Baofeng’s book of “Intervertebral Disc and its Peripheral Tissue Injury”. in the late 1970s, he also participated in the study class of Feng Tianyou’s new medical orthopaedics. At the same time, he studied the history of Chinese orthopedic technology and was well versed in the orthopedic literature of traditional Chinese medicine, and in the early 80’s, he further studied orthopedics and spinal surgery in Tianjin Hospital. In the early 80’s, he further studied orthopedics and spine surgery in Tianjin Hospital, so he is familiar with the theories and clinical skills of both Chinese and Western medicine in the diagnosis and treatment of spinal injuries and diseases. Therefore, in the clinic, no matter Western medicine surgical treatment or Chinese orthopedic massage and acupuncture treatment, can not completely solve the problem of spinal injuries and diseases. Academician Ge Baofeng once pointed out that “the etiology and pathology of spinal injuries and diseases are mainly mechanical problems.” For this reason, he deeply researched the formation mechanism of human spinal curvature, and solved the unknown mystery of spinal kinematics one by one from the biological point of view. Prof. Wei mainly from two aspects of the spinal disc analysis and research: (a) the intervertebral disc itself has a power structure biomechanical research shows that, in the process of spinal movement is relying on the synergistic effect of muscles and nerves, the intervertebral disc in a complex way to bear the load, the lumbar spine of the lumbar spine of the lumbar vertebrae flexion and extension and lateral flexion of the intervertebral disc can be produced by tensile and compressive stress, and lumbar spine rotational disc is mainly subject to the force of shear stress. The intervertebral disc, as a water-rich tissue, acts as a cushion between the vertebral bodies throughout the movement phase. The intervertebral disc only functions as a hydrostatic device, storing and transferring loads. Prof. Yizong Wei’s research shows that the hydrostatic force of the intervertebral disc originates from the formation of the vertebral curvature. The growth and development of the intervertebral disc originates from the notochord. After the formation of the body segments, the notochord cells remain to form the nucleus pulposus, which is stabilized between the vertebrae throughout the development of the spinal column up to 6 months after birth. During this stage of development, all nuclei are static. When children start to sit and stand at 6 months, lumbar curvature appeared, to one year old standing and walking, cervical curvature appeared, cervical lumbar curvature appeared, the intervertebral space appeared in front of the wide after narrowing, the nucleus pulposus in the vertebral body under the action of the stress, was pushed to the front, so that the intervertebral space appeared in the original nucleus pulposus when newborn space, this space is gradually filled with water, the nucleus pulposus began to have the vertebral body under the movement of the hydrostatic force. This shows that the intervertebral disc itself has no power structure. (b) Why the disc can “protrude”, Wei Yizong’s research once again confirmed that the vertebral body movement is the main driving force of disc protrusion. Nucleus pulposus and its connection to the upper and lower vertebrae of the annulus fibrosus composed of intervertebral disc is closely connected to the upper and lower vertebrae, therefore, any movement of the vertebrae can be driven by the disc’s movement, that is to say, there is no movement of the vertebrae, the disc can not be herniated on its own. Once the intervertebral disc herniation, the intervertebral space becomes narrow, the vertebral body collapses and rotates, the articular synovial joints must become angular interlocking and affect the interlocking of the upper and lower articular synovial joints, and the vertebral body tilts and rotates, and twisted scoliosis occurs, which is the mechanism of the imaging changes of the straightening of the vertebral curvature and the upper lumbar vertebrae rotating, bending, and tilting that occur almost all the time after the intervertebral disc herniation and the narrowing of the intervertebral space. On the other hand, due to the vertebral curvature disorder, lumbar lateral bending, tilting, lumbar spine in the longitudinal axis load stress, longitudinal axis force line displacement, not only aggravate the herniated disc site of the joint stress compression, but also can be secondary to the last disc due to the tendency to herniate under the action of compressive stress. This is often clinically lumbar 5 sacral 1 disc herniation, and then secondary lumbar 4, 5, lumbar 3, 4 disc herniation mechanism. In short, herniated intervertebral disc is the result of degeneration of intervertebral disc and uncoordinated movement of the spine. Third, Wei Yizong pioneered spinal diagnosis and treatment of vertebral curvature theory formation Professor Wei Yizong led a team of experts to use cadaveric anatomy, animal experiments, X-ray photo dynamic observation and clinical analysis, to conduct a series of manipulation of kinematics, biomechanics research, such as in the pelvic traction and the side of the plate under the lumbar intervertebral disc negative pressure dynamic test; over-extension traction on the lumbar muscle and the lumbar vertebrae structural mechanics, kinematics of the impact of cervical, lumbar vertebrae, rotational method, lateral trigger method X-ray. Rotational method, lateral trigger method X-ray dynamic study and so on a series of experiments; at the same time, learning from past lessons, mastering the strict indications, contraindications and operational precautions. At the same time, we have learned the lessons from the past and mastered the strict indications, contraindications and operation precautions. We put forward the famous vertebral curvature theory, which is the scientific principle of treating both the disease and the human being, and it has made the spine diagnosis and treatment undergo fundamental changes. On September 16, 2002, Bai Xiaoyun, a reporter of China Traditional Chinese Medicine Newspaper, reported for the first time on the vertebral curvature theory put forward by Prof. Wei Yizong with the title of “Uncovering the Mystery of Chinese Medicine Rectification”; and in June, 2004, on the “4th World Orthopaedic Chinese Medicine Academic Exchange Conference” in Mainz, Germany, Prof. Wei Yizong was invited to attend the “4th World Orthopaedic Chinese Medicine Academic Exchange Conference”. In June 2004, Prof. Wei Yizong’s academic speech on “Chinese Chiropractic’s Vertebral Curvature Theory” at the “4th World Congress of Chinese Orthopaedic Scholarship” held in Mainz, Germany, aroused a great sensation among the international medical colleagues. At the request of German scholars, Prof. Wei Yizong expounded the Chinese chiropractic theory of vertebral curvature to his European counterparts in the “Chinese chiropractic” study class held after the conference. For the first time, Prof. Wei openly challenged the intervertebral disc dynasty in an international forum. 2012 July, the 9th World Orthopaedic Conference of Chinese Medicine was held in Chicago, U.S.A., and Prof. Wei Yizong’s Chinese chiropractic vertebral curvature theory aroused the great interest of more than 100 U.S. physicians. In December 2013, the U.S. government “Acupuncture and Oriental Medicine Accrediting Agency (NCCAOM), the Chinese chiropractic vertebral curvature based on the basic theory and techniques approved for inclusion in the U.S. New York University of Health Professions Continuing Education Program, to start learning classes. With the release of the English version of Chinese Chiropractic in 2010, more and more scholars have recognized Ezra Wei’s theory of vertebral curvature. Professor Wei Yizong used the functional anatomy of the human spine as the entry point to study the mechanism of human cervical and lumbar curvature formation, and discovered the important role of the psoas major muscle in the formation of lumbar curvature, and came to the conclusion that “the lumbar vertebrae are not only the basis of the structural mechanics of the entire spine, but also the basis of the mechanics of movement”. Vertebral curvature determines the size of the spinal canal, the size and orientation of the nerve root foramen, and the course of the cervical vertebral artery. Disorders of the vertebral curvature originate from an imbalance in the four-dimensional musculature (anterior two-dimensional psoas muscle, one on each side of the anterior part of the lumbar vertebral body, and posterior two-dimensional erector spinae muscle, one on each side of the posterior part of the lumbar vertebral body), which is mainly responsible for maintaining the lumbar vertebral curvature. The pathology of spinal injuries centers on vertebral body displacement, changes in vertebral curvature, and concomitant foraminal and canal deformities. Mechanical disorders of the vertebral curvature are the main cause of spinal disorders, so chiropractic adjustment of the vertebral curvature so that the curvature is restored is the key to the treatment of spinal disorders. Using the treatment concept of bone-setting, i.e., the right position, the right line, and the right axis, which is extended to the treatment of spinal strain disease, the adjustment of the curvature becomes the main treatment goal of Chinese Chiropractic, and the “theory of the curvature of the vertebrae” becomes the core theory to solve the problem of spinal mechanics. For this reason, he researched and clarified the theories of a circle, a statement and quadrilateral balance, all of which are for the service of the “vertebral curvature theory”. Accordingly, he proposed that “the cause of cervical spine osteoarticular disorder due to cervical spondylosis is not in the cervical vertebrae”, “the pain caused by lumbar disc herniation is not only due to the compression of nerves by the intervertebral discs”, “the main causative factor for lumbar spondylolisthesis is the disturbance of the vertebral curvature. The main cause of lumbar spondylolisthesis is vertebral curvature disorder”, “degenerative spinal stenosis is dynamic stenosis” and other new theories of etiology and pathology. In view of the etiology of spinal disease, Prof. Wei invented and obtained the national patent of “wall-mounted four-dimensional chiropractic traction bed”, which adopts the over-extension suspension traction method, which is the over-extension traction method mainly used for traction of both lower limbs, and it fully mobilizes the lumbar large muscles to stretch the spine. Through four-dimensional suspension traction, the displaced vertebral body can be reset to achieve the “tendon soft bone”, so that the disordered vertebral curvature can be restored or improved. This will lead to the restoration or improvement of the disordered vertebral curvature and achieve satisfactory clinical results. Once the vertebral curvature changes, it is due to the displacement of the vertebral body and the synovial joints. So driven by the intervertebral disc fibrous ring distortion or tear, disc herniation, nerve root, vertebral artery damage, severe spinal stenosis compression of the spinal cord [8]. Observation of cervical and lumbar spinal curvature has become an objective indicator of clinical diagnosis. In terms of treatment, if the vertebral curvature can be adjusted to recover, the symptoms and signs disappear with it, and, moreover, the recurrence rate is low. Cervical and lumbar vertebral curvature is both the physiological manifestation and the basis of pathology, the basis of diagnosis and the goal of treatment. The only vertebral curvature theory makes it clear that the chiropractic treatment principle is to adjust the curvature. The discovery and application of the vertebral curvature theory has brought about a qualitative change in the chiropractic clinic, which has improved the diagnostic level of chiropractic clinic and treatment effect, and has provided the objective indexes of diagnosis and efficacy assessment of non-surgical treatment by imaging. It has changed the chaotic situation that there is no objective index for evaluating the efficacy of non-surgical treatment of spinal strain and spine-related diseases in the whole world orthopedics. The formation of the “vertebral curvature theory era” of spinal diagnosis and treatment created by Prof. Wei Yizong The publication of the monograph “Chinese Chiropractic” in 2006 marked the formation of modern Chinese chiropractic. At the same time, the State Administration of Traditional Chinese Medicine, the Chinese Association for Science and Technology and the Ministry of Civil Affairs approved the establishment of the Chiropractic Branch of the Chinese Society of Traditional Chinese Medicine, which gave Chinese chiropractors their own academic organization. In just 6 years, Chinese chiropractic in the Chinese Academy of Chinese Medicine chiropractic branch of the leadership of Professor Wei Yizong has published “chiropractic series of college textbooks”, “Professor Wei Yizong chiropractic atlas,” “Chinese chiropractic・second edition”, in September 2009, the State Administration of Traditional Chinese Medicine Policy and Regulations Division of the official project issued to establish the “Chinese medicine chiropractic diagnosis and treatment of common diseases guide” standardization task, respectively, in September 2009 and April 2010, the State Administration of TCM and the Ministry of Civil Affairs approved the establishment of the Chinese chiropractic branch of the Chinese Academy of Chinese Medicine. In September 2009, the Department of Policies and Regulations of the State Administration of Traditional Chinese Medicine formally set up a project and issued the task of establishing the standardization of “Guidelines for the Diagnosis and Treatment of Common Chinese Chiropractic Conditions”. And senior experts were invited to validate, revise and finalize the draft, and in December 2010, the National Committee of Experts on Standardization of Traditional Chinese Medicine held a discussion meeting, which was passed in principle. In June 2011 in Chaozhou City, held the “Seventh Annual Meeting of Chinese Medicine Chiropractic Academic Conference”, began to promote within the Society, in September 2012 again final review, on October 13, 2012 was officially released to the community. The chiropractic technology has become a national standard for the promotion and implementation of the treatment of spinal diseases. Incorporated chiropractic spinal health care practitioners into the revision of the National Occupational Canon within it, and has completed the research and review of chiropractic occupations. From 2003 to June 2014, a total of fifty-two national chiropractic advanced training courses have been held, and more than 5,000 Chinese chiropractors have been trained throughout the country and overseas. In this way, there are college textbooks, chiropractic discipline standards, discipline personnel construction system, clinical application, forming a complete discipline system. Fifth, Wei Yizong spine diagnosis and treatment of vertebral curvature theory of the era of practical significance Professor Wei Yizong created to solve the spinal strain disease of innovative diagnosis and treatment system, in the past 10 years by the country’s 15 scientific research collaborative hospitals, so that more than 60,000 cases of serious cervical and lumbar spondylolisthesis, intervertebral disc protrusion, lumbar vertebral slip, spinal stenosis, adolescent scoliosis, and other difficult spinal diagnosis and treatment of the patient to get well, to avoid surgical pain or disability, has been the State Administration of Chinese Medicine. At present, it has been established by the State Administration of Traditional Chinese Medicine as the core therapy of “National TCM Spine Diagnosis and Treatment Chiropractic Guidelines”. Since its promotion in 2006 to 2013 statistics, more than 200 hospitals across the country to implement the vertebral curvature of chiropractic, Beijing Guangming Orthopaedic Hospital, Guangdong Provincial Hospital of Traditional Chinese Medicine and Gansu Provincial Hospital of Traditional Chinese Medicine and other 15 hospitals, over the years, with the “Guide” chiropractic treatment of previous need for surgical treatment of herniated intervertebral discs 33,577 cases, cervical stenosis 345 cases, lumbar spinal stenosis 11,037 cases, 541 cases of lumbar spondylolisthesis, and 460 cases of adolescent scoliosis, totaling 45,960 cases. The cost of surgical treatment for these cases of herniated discs was three times that of chiropractic care, the cost of surgical treatment for adolescent scoliosis was 10 times that of chiropractic care, and the cost of surgical treatment for the other diseases was five times that of chiropractic care. According to this calculation, only the above 15 units over the past few years to treat 5 difficult spinal diseases will save billions of dollars in medical costs, and make the patient free from the pain of a knife, for the Chinese medicine diagnosis and treatment to open up new horizons. Prof. Wei Yizong, based on the Chinese elements of the I Ching, the laws of the round way of cosmic movement and the natural view of the unity of heaven and mankind, applies holistic thinking instead of fragmentary thinking, systematic thinking instead of mechanical thinking, and dynamic thinking instead of static thinking. Through scientific verification, he revealed the mechanism of human cervical and lumbar vertebral curvature and its interrelationship, and created the “vertebral curvature theory”. Prof. Ding Zhaozhong, a Nobel Prize winner and a famous physicist, said, “Science is a matter of majority rule, and only after the minority overthrows the majority’s ideas can science move forward. Therefore, expert review is not absolutely useful because expert review relies on existing knowledge, while scientific progress is to overthrow existing knowledge.” There are many scientific inventions and creations in history that were not recognized by contemporaries but recognized by later generations. The era of vertebral curvature theory pioneered by Wei Yizong has come and gone, and will ultimately become the goal of human medicine’s quest to diagnose and treat spinal injuries and diseases.
