Where is the base of the fifth topoid bone?

The base of the fifth metatarsal is located in the lateral-most part of the center of the foot, midway between the outermost little toe and the heel of the foot. This area is prone to avulsion fractures, mainly due to the attachment of the peroneus brevis tendon to the base of the fifth metatarsal. After a walking sprain, the peroneus longus shortus muscle pulls violently, resulting in partial avulsion of the bone at the base of the fifth metatarsal, which leads to hemorrhage, pain, edema, and impaired walking. Smaller avulsion fractures can be fixed by manipulation and casting, but for more displaced fractures surgical internal fixation is recommended to maintain foot stability and avoid pain during walking. The location of the base of the fifth metatarsal and the fracture can be determined by X-ray, which is usually located on the lateral side of the dorsum of the foot at the distal end of the center.
