There is no latest therapy for bronchiectasis, and its main treatment principles are treatment of underlying diseases, control of infection, improvement of airflow limitation, removal of airway secretions, hemoptysis treatment, surgical treatment and so on. 1. Treatment of underlying diseases: If bronchiectasis is caused by active tuberculosis, active application of anti-tuberculosis drugs, such as isoniazid, rifampicin, pyrazinamide and other anti-tuberculosis treatment is needed. 2. Infection control: when there is an increase in the amount of sputum and purulent components, it is necessary to actively treat bacterial infections. Especially in the acute exacerbation period, sputum culture can be carried out before treatment, and at the same time, empirical medication can be started immediately, and antibiotics such as ceftazidime and cefepime can be used. When the empirical medication is ineffective, then according to the results of drug sensitivity to choose sensitive drugs for treatment. 3. Improve airflow limitation: When obstructive ventilation dysfunction occurs, long-acting bronchodilators such as tiotropium bromide are usually applied to help improve airflow limitation and to help clear secretions. 4. Clearing airway secretions: physical expectoration can be carried out by lowering the head and buttock elevation, patting the back; or inhalation of hypertonic saline, acetylcysteine and other mucolytics can be used to dilute sputum and help expectoration. 5. Hemoptysis treatment: small amount of hemoptysis can be taken orally, such as carbachol; moderate bleeding, need to be given intravenously, such as posterior pituitary hormone; large amount of blood, if the internal medicine treatment is ineffective, can be surgical treatment. 6. Surgical treatment: when adequate medical treatment is still recurrent, surgical resection of diseased lung tissue can be considered.
