What can cause calf cramps during sleep?

  Many people have experienced calf cramps during sleep at night. Calf cramps are actually abnormal neuromuscular excitations that cause leg muscles or muscle groups to spasm, when the pulling strength of the muscle bundle is significantly greater than that of the muscle bundle during normal muscle contraction, manifesting as a sudden, violent, involuntary contraction of one or more groups of leg muscles. Although the cramp lasts only a few minutes, the discomfort or tenderness in the muscles can last for several hours after the episode. The most common clinical condition is calf gastrocnemius (calf stomach) cramps in the elderly, and they often occur at night while sleeping, and continuous episodes can interfere with sleep and reduce quality of life. Nocturnal episodes of calf cramps are related to the following factors: 1, fatigue: fatigue caused by excessive exercise or overexertion of the legs during the day, the state of muscle tension is not improved at night, excessive metabolites are not metabolized in time, and its stimulation can cause calf cramps.  2, cold: at night when sleeping, cold feet and legs can cause leg muscle cramps.  3, hypocalcemia: When hypocalcemia occurs in the body, the excitation threshold of the neuromuscular junction is lowered, and the intensity of stimulation below normal can make the neuromuscular excitation, thus it is very easy to produce abnormal contraction and cause calf cramps. This situation is mostly seen in pregnant women and elderly people with calcium deficiency.  4, blood flow factors: for patients with varicose veins or deep vein thrombosis, if they sleep for a long time to maintain a certain posture, the leg veins are pressurized, the reflux is blocked, resulting in blood stagnation, when the blood stagnation reaches a certain level, it will lead to leg muscle spasm.  There are many reasons why calf cramps occur at night during sleep, most of which are idiopathic. However, the probability of leg cramps increases with certain diseases called secondary leg cramps, such as multiple muscle diseases, peripheral neuropathies, uremia, diabetes, thyroid disease, low magnesium, low calcium, and low potassium. Most patients are of course associated with calcium deficiency. For example, postmenopausal women, people who are dieting to lose weight, etc., suffer from leg cramps due to insufficient absorption or intake of calcium. In addition, taking certain medications can also induce secondary leg cramps. These medications include hormones, morphine for pain relief, cimetidine for stomach problems, diuretics and nifedipine for lowering blood pressure, statins for lowering cholesterol, and lithium salts for psychiatric use.  Regardless of the cause of calf cramps, they can produce discomfort or pain that interferes with normal sleep.  Therefore, it is necessary to pay enough attention to eliminate the factors that can produce calf cramps: (1) Before going to sleep, avoid drinking alcohol, coffee and cola and other beverages that have a stimulating effect on the central nervous system, and do not read books that have a stimulating effect, do not watch stimulating films and videos, and do not make a fuss until you feel like sleeping and can ensure that you go to sleep soon after lying in bed, then go to bed. When sleeping, pay attention to keep warm and massage the lower limbs, etc.  (2) If calcium intake is insufficient, give priority to calcium supplementation, preferably taken before bedtime.  (3) When calcium supplementation does not relieve cramping symptoms, try vitamin E supplementation twice a day for 2 weeks. If symptoms are reduced, change to once daily.  (4) If neither calcium nor vitamin E supplementation is effective, try magnesium and potassium supplementation (5) Others have tried the vasodilator drug nafuramidine, anticholinergics, calcium antagonists, and antiepileptic drugs. These medications should be taken under the supervision of a physician. If seizures are more frequent, you should seek medical attention to identify the cause and treat it early.
