How to treat gynecologic leukoplakia

Gynecological leukoplakia generally refers to vulvar leukoplakia, which can be divided into vulvar chronic simple tinea versicolor and vulvar sclerosing tinea versicolor according to histopathological features. It can be treated by general treatment, medicine, physical, surgery and so on according to the doctor’s instruction.
1. General treatment: the specific cause of this disease is unknown, may be related to chronic friction and irritation, so patients should keep the local skin clean and dry, avoid wearing impermeable chemical fiber underwear, in addition, should avoid the use of stimulating drugs or cleansing agents to wash the vulva.
2. Drug treatment:
(1) Vulvar chronic simple tinea: can follow the doctor’s instructions to use fluoxetine, tretinoin, hydrocortisone, etc., to control the itching, but long-term use may make the local skin atrophy.
(2) Vulvar sclerosing tinea: glucocorticosteroids such as clobetasol propionate can be used as prescribed by the doctor to relieve the discomfort; if topical hormonal drug treatment is ineffective or the side effects are hard to tolerate, drugs such as tacrolimus can be considered. Oral retinoids can also be taken to maintain the normal function and structure of the epithelium and mucosa.
3. Physical therapy: If the condition is serious or the medication is ineffective, physical therapy, such as carbon dioxide laser, focused ultrasound, etc., can be used to remove the local abnormal epithelial tissues and destroy the nerve endings of the dermis layer to alleviate the discomfort.
4. Surgical treatment: For those who are ineffective in non-surgical treatment, those who have atypical hyperplasia or have the possibility of malignant changes, etc., surgical removal of the lesions can be considered after evaluation by the doctor.
