Genetic counseling, also known as genetic negotiation, is the process of advising a person with a genetic disorder or a relative at risk of developing a genetic disorder on the regression of the disorder, the probability of its onset or inheritance, and methods of its prevention or mitigation. The purpose is to limit the birth of children with genetic diseases through counseling in order to reduce the frequency of genetic diseases and improve the quality of the population. Its program generally includes counseling on the cause, mode of inheritance, diagnosis and prognosis of a genetic disease. Its task is to predict the offspring, determine the risk rate of the patient’s siblings and children to suffer from the same disease again, and provide recommendations and guidance for the patient or family. It is a means of preventing hereditary diseases. Natural reproduction is the process by which, after sexual intercourse, sperm and egg unite in the fallopian tube to produce a new generation of individuals. Conception is a physiological process and is extremely easy for couples of normal reproductive age to conceive if they have a normal sex life, but there are some couples who have been married for many years without conceiving and have not succeeded despite seeking medical help. So, what are the necessary conditions to successfully complete the process of conception? It includes having “good seeds”, “smooth roads”, “good opportunities” and “fertile soil”. four conditions. As for the “good seed”, a woman must have ovaries that can produce normal and healthy eggs, and a man must have testicles that can produce normal and healthy sperm. The timing of sexual intercourse is particularly important because sperm can only survive for 6 hours in the vagina, and the life span of sperm that can travel up into the fallopian tube is only 1-3 days, and the real sperm with the ability to fertilize only lasts for about 20 hours. The life span of an egg is usually only about 24 hours, and the time with the ability to fertilize is only 12 hours. Therefore, if you choose to have intercourse on the day of ovulation, it is possible to get pregnant. The so-called “fertile soil” is the fertilized egg, which arrives in the uterus from the fallopian tube after about 3-4 days, when the endometrium becomes thick and soft, just like a fertile soil, rich in nutrients. The fertilized egg is planted in this endometrium and gradually develops into an embryo and fetus. If inbred persons or those with serious genetic diseases, and other subjects unfit for marriage, are allowed to reproduce naturally, there is a risk of birth defects. Therefore, in order to prevent harmful heredity and improve the quality of the population at birth, the above couples should undergo the necessary genetic counseling and make scientific reproductive choices under the guidance of a doctor, and some should consciously abandon natural reproductive methods and use reproductive engineering to have children. Some couples are emotionally and psychologically disturbed after the birth of a child with a congenital defect or the death of a newborn, and some well-meaning friends and relatives often urge couples to have another child early to make up for the defect or the loss of a child out of sympathy, which is detrimental to both the couple itself and to the family and society, because what is needed at this time is physical and psychological recovery. During this time the couple should visit a genetic counseling clinic and provide a detailed and accurate medical history, as well as the composition of the immediate and multiple family members within at least three generations and the onset of the disease. A genealogical analysis is performed through the mapped pedigree, followed by a comprehensive analysis of the patient’s clinical, chromosomal and/or biochemical findings, and family history of morbidity before making a diagnosis and calculating the risk rate of morbidity in order to give counseling clients options for fertility.
