Abstract: Inflammatory polyps of the small intestine are a type of small intestinal polyp that is relatively mild, can be solitary or multiple, and can cause symptoms such as abdominal pain and black stools. In this case, the patient presented to the hospital with symptoms such as abdominal pain, black stool and blood in the stool, and the diagnosis of inflammatory polyps of small intestine was confirmed by endoscopy and other examinations. After treatment, the patient’s condition was effectively controlled and the stool returned to normal after 1 month, and the polyp was completely removed.
Basic information】Male, 58 years old
Disease Type】Inflammatory polyps of small intestine
Hospital】The First Affiliated Hospital of Kunming Medical University
Date of consultation】March 2022
Treatment plan] Endoscopic mucosal resection, anti-infection treatment (penicillin sodium) and rehydration support, etc.
Treatment period】3 days of inpatient treatment, 1 month of outpatient follow-up
Results】The disease was effectively controlled, stool returned to normal, and polyps were completely removed.
I. Initial consultation
Two months ago, the patient came to the clinic with abdominal pain, black stool and blood in the stool. The endoscopic examination indicated mucosal redundancy in the small intestine, and the tumor marker examination showed no abnormalities. The patient had no past genetic history, smoked for more than 30 years, and drank alcohol from time to time. The patient was initially diagnosed as small intestinal polyp and was admitted to the hospital.
II. Treatment history
After admission, the patient continued to improve blood glucose, lipids, liver and kidney function, C-reactive protein and other related tests, the results were not significantly abnormal, fecal occult blood test showed positive, X-ray barium enema can be seen slightly round or oval filling defect, smooth and clear margins, combined with the results of colonoscopy, basically confirmed the diagnosis of small intestinal polyps. After discussion with the patient, endoscopic mucosal resection (EMR) was performed on the second day of admission to remove the small intestinal polyp, and the specimen was sent for pathological histological examination, which showed that it was an inflammatory polyp of the small intestine. After the operation, the patient did not have any discomfort and returned to the ward, and was given routine fasting and water. Since the neutrophil percentage would increase after the operation, he was given intravenous penicillin sodium for injection, as well as rehydration and nutrition support. The patient’s condition was relatively stable, and he did not complain of abdominal pain, fullness and other discomforts, and was discharged on the third day of admission after comprehensive evaluation.

III. Treatment effect
After giving surgical and medication treatment, the patient recovered well, and the abdominal pain symptoms were significantly relieved until they disappeared. The patient was asked to review the patient’s condition after 1 month. The patient’s mental status was good, stool was back to normal, the polyp was completely removed by endoscopy, the surface of intestinal mucosa was smooth and soft, the mobility was good, and the condition was under control.
IV. Notes
As a treating physician, we are very happy that the patient’s disease has improved after treatment, the stool has returned to normal and the abdominal pain has disappeared. However, the patient was advised to take medication as prescribed by the doctor after discharge and to review regularly to prevent recurrence. Observe yourself carefully in the short term after surgery, and if blood in the stool, dark stool and abdominal pain occur, seek medical attention to clarify the diagnosis. After surgery, adjust your work and rest, avoid overwork, do not engage in strenuous activities, quit smoking and drinking, avoid spicy, stimulating and cold food, eat small and frequent meals, do not eat too fast, and ensure sufficient chewing to reduce the burden on the stomach and intestines, and eat less fried and indigestible food.
V. Personal insight
For inflammatory polyps of small intestine, generally after treatment can be cured, but patients still need to follow up regularly after surgery to avoid its recurrence. Most patients do not have clinical symptoms when the polyps are small, so regular checkups are needed for earlier detection and early treatment. No matter what kind of disease patients suffer from, they will be nervous and afraid of the disease, thus creating the psychology of avoiding the disease, which will eventually lead to the aggravation of the disease and increase the difficulty of treatment, so they should adjust their mindset and strengthen the preventive health care of their body.
