Goiter is a common and frequent surgical condition with a prevalence of up to 4%, including thyroid adenoma, nodular goiter, and thyroid cancer. Surgery is an effective treatment for these thyroid disorders. However, the majority of patients with thyroid disease are women, with a male to female ratio of 1:3-7, and most patients are young women. Although traditional thyroid surgery is not very invasive, it leaves an unsightly 6cm to 10cm long surgical scar on the front of the exposed neck, which causes strong psychological stress to the patient. Minimally invasive and minimally scarred modified thyroidectomy basically meets the aesthetic requirements of most patients, but still does not reach the highest level of a completely scarless neck. With the development of high-tech instruments and the increasing pursuit of beauty, surgeons have begun to explore the application of lumpectomy to thyroid surgery in order to achieve the exact therapeutic results and the best aesthetic, minimally invasive results. This has led to the development of incisionless lumpectomy of the neck. The lumpectomy thyroid surgery takes advantage of the magnification of the lumpectomy image and the use of ultrasonic knife to cut the gland and thyroid blood vessels to avoid bleeding, and the remote operation with slender surgical instruments has changed the tradition of thyroid surgery requiring a direct incision in the neck lesion, which not only makes the surgery safer, but also makes the surgical incision only the size of a “keyhole”. The incision is only the size of a “keyhole” and is transferred to a hidden part of the body, achieving the aesthetic effect of no surgical scars on the exposed neck. The world’s first lumpectomy was performed by Huscher in 1997, and the technique was introduced in China in 2001, and has been developed in parallel with the international advanced level. In 2005, our department was the first to perform lumpectomy in the province, filling the gap of this high-tech field in the province. As of October this year, our department has successfully completed more than 200 cases of lumpectomy thyroid surgery, which has been appraised by authoritative lumpectomy experts and has reached the leading level in China and won the Science and Technology Innovation Award. The lumpectomy thyroidectomy is of great cosmetic value, so it has shown great vitality as soon as it appeared, and is favored by patients, especially young women. The main advantages of lumpectomy thyroid: (1) the same treatment effect as traditional surgery; (2) less trauma, less bleeding and faster recovery due to the use of ultrasonic knife and minimally invasive instruments; (3) less chance of nerve, blood vessel and parathyroid gland damage due to the magnifying effect of lumpectomy; (4) greater advantage is that the neck is completely scarless and the patient’s privacy can be protected to the maximum. The tiny incision in the anterior chest leaves almost no scar after healing and can be completely covered by undergarments. Current indications for lumpectomy include: unilateral or bilateral benign adenomas, cysts, and nodules; hyperthyroidism with normal or mildly enlarged gland size; papillary thyroid cancer less than 1 cm in diameter, no surface invasion of the gland, and no enlarged lymph nodes adjacent to the central group of carotid arteries. Contraindications include: history of surgery or radiotherapy to the neck; suspected malignant mass; large multiple nodules; thyroiditis; pathological obesity; and coagulation disorders. Our department is also the largest thyroid treatment center in central China. In addition to the characteristics of lumpectomy, we also carry out the combination of Chinese and Western medicine and radiofrequency ablation to treat various thyroid diseases (thyroid tumor, hyperthyroidism, thyroid cancer, parathyroid adenoma, parathyroid cancer, etc.), combining the identification of evidence and disease, giving full play to the advantages and characteristics of Chinese medicine treatment, and actively applying the diagnosis and treatment methods of modern medicine. We have solved the medical problem of persistent and recurrent neck diseases, which is generally welcomed by patients.
