Abstract: Vasodilatory rhinitis is a non-infectious, non-allergic inflammatory disease of the nasal mucosa, which mainly manifests clinically as itchy nose, nasal congestion, runny nose, sneezing, etc. In this article, the patient came to the clinic with “nasal congestion, sneezing and runny nose for 20 years, aggravated for 1 month”, which was not treated. Through examination, she was diagnosed with vasodilatory rhinitis and deviated nasal septum, and was advised to use medication + surgery to treat the symptoms of nasal congestion and runny nose.
Basic information】Female, 42 years old
Disease Type】Vasodilatory rhinitis, deviated septum
Hospital】Northern War Zone General Hospital
Date of consultation】September 2021
Treatment plan】Medication (tretinoin nasal spray, nasal spray of azelastine hydrochloride) + surgery (nasal septum correction)
Treatment period】Hospitalization for half a month, and review after half a month
Treatment effect】The symptoms of nasal congestion and runny nose disappeared, ventilation improved significantly, and recovery was good.
I. Initial consultation
One day in September 2021, a female patient came to the clinic. The patient reported that she had been suffering from nasal congestion for 20 years, which was aggravated at night and often affected her sleep by blowing her nose, and often caused headaches due to nasal congestion and dizziness and dizziness due to poor rest. Next, the patient said that in the past 1 month, he had symptoms of itchy nose and sneezing without any obvious cause, and the runny nose was more than before, so he came to the clinic. On examination, he found that the nasal septum was obviously deviated, the mucous membrane of the nasal cavity was congested, the bilateral inferior turbinates were slightly enlarged, and no abnormality was found in the nasopharynx.
II. Treatment
Through the diagnosis, the patient was recommended to undergo a drug + surgery treatment. For medication, the patient was recommended to apply trimethoprim nasal spray and azathioprine hydrochloride nasal spray. Before using the nasal spray, the nasal cavity was flushed with physiological sea saline, and the two nasal sprays were used at 5-10 minutes intervals to help reduce the sensitivity of the nasal mucosa, reduce the congestion of the nasal mucosa, and equip the nasal cavity for surgery. After that, he was advised to consider septal correction to improve nasal ventilation. The patient accepted this treatment plan and actively cooperated with the surgical treatment.
III. Treatment results
The patient’s symptoms of nasal congestion, sneezing and runny nose had improved significantly after the operation, and the ventilation was significantly improved. After half a month of hospitalization, the patient was discharged from the hospital, and the symptoms of nasal congestion and runny nose disappeared and the ventilation was good. The patient could work and live normally.
IV. Notes
The patient’s result after treatment is really happy. After discharge, take the medication regularly and quantitatively, avoid stopping it by yourself, and if there is any discomfort or symptom aggravation, it is recommended to consult a doctor for follow-up. In addition, it is also necessary to pay attention to home care, pay attention to environmental cleanliness and hygiene at home and work place, and keep appropriate humidity; avoid bad habits such as digging nose, and do not blow nose hard. In daily life, exercise appropriately to improve resistance; avoid spicy and stimulating diet, which will aggravate the runny nose.
V. Personal insight
Vasodilatory rhinitis is a common disease in ENT clinic, and the symptoms are especially similar to allergic rhinitis and upper respiratory tract infection. First of all, we need to make a clear diagnosis and actively use symptomatic medication so that the clinical symptoms of patients can be improved significantly, and usually we need to ensure a positive state of mind, avoid contact with hot and cold air stimulation, avoid contact with dust, and exercise more to improve resistance, which is also beneficial to the recovery of the disease. If the effect of conservative treatment is not good, or if the patient has obvious deviation of the nasal septum and nasal turbinate hypertrophy, surgery can be considered for treatment.
