Parkinson’s disease drug treatment and strategies

  Parkinson’s disease is a chronic progressive neurodegenerative disease of the middle-aged and elderly due to nigrostriatal dopaminergic neuron lesions. The main clinical manifestations are tremor, myotonia, bradykinesia and postural disturbances. Although the development of PD has been found to be related to iron and calcium overload and oxidative stress in recent years, the exact cause of PD is still unclear and there is no cure so far. Given that PD is mainly caused by degeneration and loss of nigrostriatal dopaminergic neurons, resulting in a significant reduction of striatal dopamine, the main idea of treatment is to increase the dopamine content in the central nervous system; to correct the imbalance between the functions of the two systems of dopaminergic and cholinergic nerves; and to protect and restore the degenerated dopaminergic and other related neurons. Since surgery is strictly indicated and expensive, and gene therapy and stem cell therapy are still in the research stage, pharmacotherapy remains the main treatment for PD. In this paper, we review the treatment strategies and progress of drug therapy for PD.  1, the principles of drug therapy for PD The main purpose of PD treatment is to slow down the progression of the disease, control the symptoms of the disease and improve the quality of survival of patients. It is generally believed that the goal of treatment for young, early patients is to maintain or restore the ability to work; for patients in the middle and late stages is to maintain or restore the ability to take care of themselves; for patients with advanced PD is to reduce pain and prolong life. Its treatment principle should emphasize comprehensive treatment, including drugs, physical therapy, medical sports, tai chi, sword fighting, qigong and daily life adjustment. Now anti-PD drugs are allopathic drugs and need to be taken for life, most of them can cause certain adverse reactions, so the clinical application of anti-PD drugs should be titrated. In 1998, the Society of Neurology of the Chinese Medical Association recommended the following principles of drug therapy: long-term medication, symptom control; symptomatic medication, increase and decrease; minimum dose, best effect, that is to say, the long flow of water, not to seek the full effect; weigh the advantages and disadvantages, the best effect. However, if the patient believes that the disease has affected him, drug treatment should be started.
