The patient was self-conscious of loose and shifted teeth and gaps between teeth. There was a history of bleeding from tooth brushing for several years, and both his father and aunt had a history of severe periodontitis. After being admitted to the hospital, relevant examinations were completed, and SRP (root planing and subgingival scraping treatment), medication-assisted treatment such as metronidazole, loose tooth fixation, periodontal maintenance and other treatments were carried out. After six months of treatment, the patient’s symptoms gradually improved and she was followed up regularly. Initial Interview In September 2021, an 18-year-old female patient came to our department because of self-consciousness of lower anterior teeth loosening for more than 1 month, and bleeding from brushing, and inquired about the medical history, the patient had a history of bleeding from brushing for a few years, and denied a history of smoking, a history of systemic diseases, and a history of drug sensitivity. His father and aunt had a history of severe periodontitis. Physical and specialized examination revealed poor oral hygiene throughout the patient’s mouth, tartar (++), malocclusion, red and swollen gingiva, loose texture, bleeding on light probing, predominantly the anterior teeth, probing revealed BI ranging from 2-4 and PD from 4-8 mm with pus inside, probing of the roots showed rough surfaces with tartar, and varying degrees of attachment loss could be detected. The looseness examination revealed that the upper anterior teeth were 12-22 degree I looseness, the lower anterior teeth were 45-35 degree II looseness, and the oral cavity surface tomography showed 12-22 bone resorption degree I and 36-46 bone resorption degree II. After completing the relevant examinations to assess the condition and test results did not see obvious abnormalities, according to the patient’s clinical manifestations, and auxiliary examinations, the patient was diagnosed as invasive periodontitis (severe extensive type). II. Treatment Given that the patient had no special contraindications, and considering that the patient was young and the symptoms of his condition were not too serious, the patient and his family were advised to treat him as soon as possible to terminate the lesions on his teeth, and a treatment plan was proposed. After the patient agreed to the treatment plan, the patient was first taken intraoral preoperative photographs, a large periodontal checklist was established, and after the patient signed an informed consent form, SRP (root planing and subgingival scraping treatment), medication-assisted treatments such as metronidazole, loose tooth fixation, and periodontal maintenance treatments and other conventional treatment means were carried out. During the treatment period, the patients were followed up by telephone and oral hygiene education, and the patients actively cooperated with the follow-up, and strictly followed the doctor’s instructions for treatment and follow-up. Under the conventional treatments such as SRP, medication-assisted treatment, loose tooth fixation, periodontal maintenance, etc., the patient’s review after six months showed that the symptoms had gradually improved, there was no bleeding symptom from brushing, tooth looseness had been effectively improved, the lesion had turned into the resting stage, and periodontal inflammation had subsided. The intervals for review were determined according to the control of plaque and inflammation. At the beginning, it is once every 1-2 months, and if the condition is stable, the interval can be gradually extended. Fourth, precautions The patient’s symptoms were effectively improved after treatment, for which I was very happy, but the patient still needs to pay attention to the following matters: 1, aggressive periodontitis is easy to recur, so it is necessary to do early detection, early diagnosis, early treatment, and regular review and adhere to the follow-up treatment, it is recommended that the patient, even if the symptoms have been relieved, but also comply with the doctor’s orders on schedule for review and adherence to the periodontal care; 2, after the surgical treatment in order to reduce the Tissue edema, you can apply ice packs to the surgical area. Prohibit sucking on the wound, try not to spit, the day of surgery need to eat warm and cool liquid food, note that postoperative saliva with light red blood is normal, 1-2 days can disappear. After normal eating, you should rinse your mouth with mouthwash in time, remove the plugging agent after 1 week, and then clean your mouth with the help of dental floss, interdental brushes and rinsers, etc.; 3, patients should consume more food rich in high quality protein, such as soybean products, meat, eggs and milk. Eat more fish rich in polyunsaturated fatty acids and vegetables and fruits rich in crude fiber to supplement multivitamins. Eat less greasy, stimulating food, such as fried food and pickled products, and ban smoking and alcohol. Fifth, personal feelings Most of the patients with aggressive periodontitis are very young, and the damage caused by the disease is very great. Therefore, we should strengthen the popularization of this disease, increase the awareness of this disease, so as to attract attention and reduce the possibility of developing aggressive periodontitis.
