Abstract: A 60-year-old patient, Uncle Zhang, went to the hospital because he had right knee pain for 2 weeks and was mistaken for arthritis, which was poorly treated with medication. 3 days ago, he developed fever and his pain symptoms were not relieved, so he came to our hospital for further examination and was finally diagnosed with Brucellosis was finally diagnosed. After the combination of medication, the body temperature returned to normal and the joint pain was relieved, and he has returned to normal life. The patient was treated with doxycycline hydrochloride capsule, rifampicin capsule, and glycopyrrolate tablets, and was hospitalized for 2 weeks, reviewed weekly, and followed up for 2 years. Patient Zhang came to the clinic with right knee pain for 2 weeks and fever for 3 days, and the routine blood test results were not abnormal. The doctor gave him topical creams and diclofenac sodium extended-release tablets orally according to arthritis, but the effect was poor. Through communication with Uncle Zhang, he learned that he had been drinking alcohol intermittently in the past and had raised sheep a year ago, feeding and cleaning the sheep on a daily basis and delivering the sheep by himself with little protection. On examination of the right knee joint, localized pressure pain was found, but there was no obvious redness or swelling, and the skin temperature was normal, so septic arthritis could be excluded. In addition, pain with fever in one joint can also exclude osteoarthritis. Based on the medical history, symptoms and signs, we considered the presence of brucellosis in Uncle Zhang, and performed brucellosis antibody test, liver and kidney function test, routine blood test, tuberculosis antibody test, blood sedimentation test and X-ray film of right knee joint, etc. The results showed that the brucellosis antibody was >1:100, suggesting positive, and there were no obvious abnormalities in other tests. The treatment was positive for Brucella antibodies and excluded tuberculosis, osteoarthritis, septic arthritis and other lesions, and there were no complications such as meningitis, myelitis, endocarditis, etc., so it was not a complicated infection. The clinical decision was made to implement the treatment plan of doxycycline hydrochloride capsule + rifampicin capsule, because these two drugs have better killing and clearing effects on Brucella, and the combined use of drugs can usually achieve a radical effect on the condition. Although the liver function was normal, the doxycycline capsule and rifampicin capsule had certain hepatotoxicity and were prone to drug-related liver injury after consumption, so it was decided to add compound glycyrrhizin tablets for liver protection treatment while anti-infection. After full communication with Uncle Zhang and his family, they expressed that they would cooperate well, and then started the treatment program. After treatment, Uncle Zhang’s body temperature returned to normal after 3 days, and his joint pain was relieved after 1 week. Within 2 weeks of hospitalization, the liver function, kidney function and blood routine were rechecked 3 times, all of which were not abnormal and met the discharge criteria. Uncle Zhang was advised to pay attention to rest and take his medication on time after discharge, and to ensure that he came to the hospital once a week to review his liver and kidney functions. After the treatment and follow-up review, after 6 weeks, Zhang’s joint pain, weakness and sweating symptoms were completely relieved, his body temperature returned to normal, and his brucella antibodies were <1:25 on retest, and his mental and physical strength returned to normal. In addition, since 10% of the patients with this disease have the possibility of recurrence during the period of 3-6 months after the end of the initial treatment, Uncle Zhang was followed up for more than 2 years, and no recurrence has occurred since the follow-up. 4. Precautions We are glad that after a series of treatments, Uncle Zhang's health has been restored and all indicators have improved, and there is no recurrence, which is inseparable from the fact that Uncle Zhang is usually in good health and brucellosis was detected in time. Although he was cured, the disease has the possibility of recurrence, so it is recommended that uncle Zhang still needs to pay attention to the following in his daily life: 1. avoid raising sheep as much as possible, if you still have the will to raise sheep, you should do a good job of personal protection, wear a mask after entering the sheep environment to prevent inhalation of aerosols contaminated by bacteria, and avoid direct contact with sheep secretions; 2. 3. Keep a good attitude, quit smoking and drinking, avoid staying up late and straining, and exercise appropriately to improve your immunity; 4. V. Personal insight Brucellosis belongs to zoonotic diseases, the onset of most of them is related to sheep farming, some veterinarians may be infected due to inadequate protection when delivering sheep, after infection often appear weakness, sweating symptoms, daily fatigue and weakness, often mistaken for laziness, so also known as lazy man's disease. The most painful symptom of the disease is muscle and joint pain symptoms, often unbearable pain, if the receiving doctor does not know enough about the disease, it can be misdiagnosed as arthritis, infectious fever, tuberculosis and other diseases, and delay the best time for diagnosis and treatment. In this article, Uncle Zhang was also mistaken for arthritis and was treated poorly, so he came to the hospital.
