Inflammation and swelling of the nail joints may be caused by nail fungus. In addition to inflammation, redness and swelling, there may be purulent oozing, pus crusting, stinging pain and itching. Clinical treatment requires routine blood tests to check for elevated total white blood cell count as well as neutrophil fraction. Blood sedimentation and C-reactive protein tests are also needed to see if the patient is in the acute response phase of inflammation. If these conditions are present, cefoxitin sodium injection or penicillin sodium injection should be administered immediately. If the total white blood cell count and neutrophil fraction are not high, and the C-reactive protein and sedimentation are normal, oral treatment with amoxicillin capsules or roxithromycin capsules and topical application of fusidic acid ointment twice daily are required.
