1.5 Drug selection in epilepsy with other disorders 1.5.1, Depression In epilepsy with depression, the preferred drugs for idiopathic generalized seizures are valproic acid and lamotrigine, and for secondary partial seizures are lamotrigine, oxcarbazepine and carbamazepine. 1.5.2, Behavioral problems For those with behavioral problems, valproic acid and lamotrigine are preferred for idiopathic generalized seizures, and lamotrigine, oxcarbazepine and carbamazepine are preferred for secondary partial seizures. 1.5.3 In renal insufficiency with renal failure requiring dialysis, valproic acid is the first choice for idiopathic generalized seizures, and the first-line drugs are valproic acid, lamotrigine and levetiracetam; the first choice and first-line drugs for symptomatic partial seizures are lamotrigine, and the first-line drugs are levetiracetam. 1.5.4 Patients with liver disease Patients with epilepsy with viral hepatitis B (hepatitis B), regardless of whether the liver function is normal or not, the preferred drug for idiopathic generalized seizures is topiramate and levetiracetam, and for secondary partial seizures with normal liver function, the preferred drug is oxcarbazepine, and in the presence of abnormal liver function indicators, the preferred drug is topiramate and levetiracetam; in patients with other liver disease, the preferred drug for idiopathic generalized seizures and In patients with other liver diseases, topiramate and levetiracetam are the drugs of choice for both idiopathic generalized seizures and secondary partial seizures. In children or elderly patients with cognitive impairment, lamotrigine is the drug of choice for idiopathic generalized seizures, and lamotrigine and oxcarbazepine are the drugs of choice for secondary partial seizures. 1.5.6 Post-stroke seizures are symptomatic partial seizures, and most of these patients are elderly. Therefore, in addition to the type of seizure, the characteristics of this special group of elderly patients should also be an important factor in the selection of drugs for the treatment of post-stroke epilepsy. Therefore, lamotrigine or oxcarbazepine is preferred in patients with post-stroke epilepsy without other systemic diseases, and is the first-line drug along with carbamazepine, levetiracetam and topiramate. Levetiracetam is preferred in those with other systemic disorders, and lamotrigine, oxcarbazepine, levetiracetam and topiramate are first-line agents. 1.6 Antiepileptic problems associated with proprietary Chinese medicines In patients with epilepsy whose seizures are controlled by proprietary Chinese medicines without adverse effects, about 1/3 of the experts believe that they need to switch to regular treatment, and for those who experience adverse drug reactions or poor seizure control, more than 2/3 of the experts believe that proprietary Chinese medicines should be discontinued. 1.7 Emergency epilepsy medications Emergency epilepsy patients are mostly unsure of their seizure type and often require rapid management by physicians. Therefore, the requirements for antiepileptic treatment in the emergency room are broad-spectrum, easy-to-use, and fast-titrating doses of medications. Expert consensus suggests that valproic acid and levetiracetam are preferred for patients with epilepsy in the emergency room, and topiramate is also available as a first-line agent.
