Intestinal obstruction in lung cancer patients may be mechanical intestinal obstruction, dynamic intestinal obstruction, hemodynamic intestinal obstruction, etc. It can be treated by gastrointestinal decompression, drug control, surgical treatment and daily maintenance. 1. Gastrointestinal decompression Gastrointestinal decompression is the basic treatment for intestinal obstruction. 2.Surgical treatment It is generally believed that surgical treatment cannot be routinely used for advanced cancer patients, and can only be selectively used for certain patients with mechanical intestinal obstruction. Some studies have pointed out that there is no difference in the mortality rate between surgical and non-surgical patients after discharge from the hospital. 3. Endoscopic treatment Endoscopic treatment includes percutaneous endoscopic gastrostomy and drainage (PEG) and endoscopic stent placement.PEG can control nausea and vomiting in 90% of patients with intestinal obstruction, and has no absolute contraindications.Relative contraindications include large tumors, portal hypertension and ascites, previous upper abdominal surgeries, active gastric ulcers, and coagulation disorders. 4.Drug treatment (1) Analgesics. According to WHO’s three-step pain relief principle can make most patients’ pain completely relieved. Strong opioid treatment should pay attention to the titration of dosage to prevent nausea, vomiting, constipation and other adverse reactions. (2) Antiemetic drugs and drugs that promote gastrointestinal motility, such as metoclopramide, can be used in patients with functional intestinal obstruction, but are not recommended in complete mechanical intestinal obstruction because they may exacerbate colic, nausea and vomiting. Other antiemetics are butyrophenones, antihistamines, and phenothiazines. Combining antiemetics that act at different sites is more effective than single agents. (3) Patients can also supplement nutrition through infusion. When lung cancer patients suffer from intestinal obstruction, it is recommended to seek medical treatment in time, be diagnosed by professional doctors and be given reasonable treatment, and do not take medicine by oneself.
