Functional exercise: Encourage the patient to take the initiative to move, concentrate the mind when practicing, breathe evenly, move accurately, do not be rough and violent, pay attention to gradually increase the activity degree and extend the time, so that the patient does not feel fatigue and the fracture site does not occur pain. (1) Straight leg raise and double lower limb bicycle method: the fracture dislocation or the first day after surgery can be exercised. (2) Upper limb abduction and expansion exercises, as well as finger, toe and ankle joint activities. Massage the abdomen in a clockwise direction 2 to 3 times a day for 5 to 10 minutes each time. (3) Functional exercise of the lumbar and back muscles ① Five-point support method: lie on your back on a hard bed, use your head, elbows and feet to support your whole body at five points, and try your best to lift your back and stretch back. For simple compression fractures without shock and other comorbidities, you can start practicing the next day after resetting, but for unstable fractures, you need to start practicing after 1 to 2 weeks of bed rest. ② Three-point support method: Lie on your back on a hard board bed, place your arms in front of your chest, support your whole body with your head and your heels, and stretch your back in the air. ③ Flying swallow point water method: the patient’s head, neck, chest and both lower limbs are lifted at the same time, the two upper limbs are stretched back, only the abdomen on the bed, the body is bow-shaped, such as flying swallow point water posture. It is suitable for practice from 3 to 4 weeks. Simple compression fracture: ① If the vertebral body is compressed less than 1/5, or the old and frail cannot tolerate the repositioning and fixation, lie on a hard board bed, with a soft pillow at the fracture site, so that the spine is hyperextended, while the casualty is instructed to start doing lumbar back muscle exercises after 3 days. At the beginning, the hips move from side to side, later flex the hips and knees, lift the hips away from the bed, and gradually transition to using both feet and head on the bed, lifting the waist and back from the bed, with the body in an arch shape. Young people wear a lumbar girth for 4-6 weeks to move on the floor, but do not bend, 3 months after the injury can be normal activities. In old and frail people, they can wear a lumbar girth for 4 weeks to move around on the ground, and cannot bend within 3 months. ② For young and middle-aged fractures with a vertebral compression height of more than 1/5, the patient should rest in bed after repositioning and do functional exercises for the lumbar back muscles, and wear a lumbar girth to move around after 6-8 weeks, and bending is prohibited for 3 months. ③ Fractures of posterior lumbar structures, such as vertebral arch fractures, spinous process fractures and transverse process fractures, can be bed rest and braking with lumbar girth, bed rest for 4-6 weeks, get up and wear lumbar girth for 2-3 months, while doing functional exercises of lumbar back muscles. Dietary care: 1.Qi stagnation and blood stasis (early stage) 1~2 weeks after fracture, blood vessels are blocked, Qi stagnation and blood stasis, pain in the chest and waist, petechiae, low fever, fatigue, irritability and dry mouth, constipation, abdominal distension and pain, poor appetite, dark red tongue with thick greasy or thin yellow greasy coating, and slippery pulse. (1) Diet is recommended to invigorate blood circulation and remove blood stasis, clear and easy to digest, such as fresh vegetables, fruits, fish fillet soup, steamed lean meat with golden needles and cloud fungus or fungus, and lean meat soup with Tian Qi. Drink more warm water. (2) For those with abdominal distension and pain and incontinent stools, give senna leaf 10-15g plus Chen Pi 5gh in water or rhubarb 60g, Yuan Ming powder 60g decoction of water 300ml to retain the enema as prescribed by the doctor. 2.Ying and Wei are not in harmony (middle stage) 3-6 weeks after injury, Qi stagnation and blood stasis gradually disappear, chest and waist abdominal distension and pain reduce or subside, petechiae gradually dissipate, tendons and bones are initially connected at the fracture, but tendons and bones are sore and soft, sometimes painful, tendons and veins are not completely open, Qi and blood are still not full of prosperity, the tongue is purple and dark, the pulse is thin and astringent. Diet is recommended to connect the bones and tendons and Ying Sheng new blood circulation diet. Such as Tian Qi chicken soup, the continuity of the pig’s feet tendon soup, Huai Shan, Coix Seed, jujube, angelica, berries, ginseng, chicken feet boiled pork backbone soup, lotus root carp chicken feet soup and so on. 3.Liver and kidney deficiency (late stage) 7-8 weeks after injury, fracture after early and mid-term treatment to remove blood stasis, tendon connection has nearly healed, but the fracture is not yet strong, waist and knee pain, flexion and extension disadvantage, limb weakness, dizziness and insomnia, light fat tongue or pointed red, thin moss, thin and slow pulse. Diet is recommended to nourish the liver and kidneys, strong tendons and bones, such as dulcimer, berries in lamb, sesame paste, black beans in chicken feet, deer tendon peanut soup, lotus root carp chicken feet soup, winter cordyceps stewed lean meat, dulcimer cow knee pork bone soup; berry yuan meat in rabbit soup: can nourish the liver and kidneys, tranquilize the mind.
