A macular cleft is a split between the anatomical layers of the retina in the macula. The split is not longitudinal across the entire retina, but rather is a split along one of the retinal layers. Macular cleavage occurs very slowly, and patients can have years of slow vision loss under constant vitreous traction with little notice. The lesion is extremely subtle and the presence of macular cleavage can only be detected on an OCT scan. Some patients can be treated with biosection + internal boundary membrane tearing surgery, the actual surgical procedure is more complicated and.
