Gonorrhea is an acute and chronic contact infection caused by Neisseria gonorrhoeae, mainly causing inflammation of the genitourinary mucosa, and is one of the sexually transmitted diseases. Since 1944, when penicillin G was used to treat gonorrhea, the incidence rate had once decreased, and after the 1960s, the incidence rate increased year by year, and the incidence rate in China has increased rapidly since the 1970s, for the following reasons: 1. Drug-resistant strains keep appearing, and in the 1970s In the mid-1970s, a strain of penicillin-resistant gonococcus with β-lactamase (PPNG for short) was found, which made penicillin treatment unsatisfactory. 2, the transmission of asymptomatic carriers, asymptomatic gonorrhea in women up to 75%, men only 1-5%. Due to incomplete treatment, symptoms may disappear, but the bacteria can still be carried. 3, changes in contraceptive methods, due to the widespread use of oral contraceptive pills, while the use of the uterine cap and condoms, which can play a role in isolating gonorrhea and other STDs. 4, the social background of sexual transmission, due to yellow books and magazines, movies and television, so that people are affected by sexual freedom, sexual liberation ideas, especially young people are the most harmful. Etiology: gonorrhea caused by Neisser’s isolated in 1879, so also known as Neisser’s gonorrhea bacterium, the bacterium is kidney-shaped, Gram-negative bacterium, often in pairs in the cytoplasm of pus cells or periphery. Transmission route: mainly through direct contact with sexual intercourse, a few indirectly through underwear, potty, tub, etc. Pregnant women with gonorrhea, newborns through the birth canal infection during delivery. Symptoms: Generally speaking, clinical symptoms occur 72 hours after infection, but weakness, excessive sexual life, alcoholism and other factors can shorten the incubation period. 1, male gonorrhea: manifested as acute anterior urethritis, redness and swelling of urethral orifice, thin mucus outflow, discomfort in urination, symptoms intensify after 24 hours, secretion of yellow pus, frequent urination, painful urination, difficulty in urination, a few cases with fever, redness and pain of lymph nodes on both sides of the groin, even pus, some appear complications such as prostatitis, epididymitis, and finally the formation of urethral stricture. 2, female gonorrhea: female patients because of the short urethra, so urinary tract symptoms are not obvious, manifested as increased leucorrhea, lower abdominal pain and other symptoms are common, so it is easy to miss the diagnosis, can be complicated by pelvic inflammatory disease, endometritis, tubal inflammation, and eventually lead to infertility. 3, neonatal ophthalmia: from the mother with gonorrhea obstetric tract infection, 2-3 days after birth appear eyelid redness, swelling, purulent discharge, if left untreated, can eventually lead to blindness, so all newborns should be routine silver nitrate eye drops. 4, baby girl vaginitis: because baby girl’s vagina is surrounded by columnar epithelium, it is very easy to be infected by gonorrhea bacterium, showing redness and swelling of perineum, purulent discharge from vagina and urethra, painful urination. 5, gonorrheal arthritis: one or several joints become septic, and pus is extracted from the joints and tested for the presence of S. gonorrhoeae, which eventually leads to bone destruction, fibrosis, and joint stiffness.
