Using Scripture to treat diabetes mellitus with headache

  The case was identified as a combination of Shaoyang and Yangming disease with heat, and Chai Hu plus Dragon Bone and Oyster Soup was chosen to relieve heat and subdue rebelliousness, pass stools and calm the mind. The beauty of the prescription in this case lies in: firstly, the use of stone cassia instead of dragon bone in the original formula, because stone cassia can not only calm the mind but also clear the liver and brighten the eyes; secondly, the use of Chinese yam, powdered kudzu root and Sichuan dendrobium instead of ginseng, cinnamon branch, poria, jujube and ginger in the original formula to build the middle jiao, which is more suitable for the patient’s condition by changing the nature of sweet and warm to sweet and flat, and the effect of Chinese yam, powdered kudzu root and Sichuan dendrobium in lowering sugar is already known to the public.  Gao, female, 53 years old, was first diagnosed on October 27, 2015. The patient has a medium physique, medium nutritional status, high blood glucose was found in the physical examination ten years ago, and she has been taking metformin to control blood glucose, and her fasting blood glucose is now stable at about 7-8 mmol/L. Now she comes to the clinic because of diabetes mellitus with headache, the patient’s headache is mainly around the temples, and the headache is aggravated by anger and accompanied by eye swelling and pain, poor sleep, weakness, dry stool once every three days, red tongue, thin yellow coating, and string pulse.  The prescription was based on the following formula: Chai Hu plus Oyster Soup: 10 grams of Northern Chai Hu, 30 grams of raw oyster, 10 grams of Radix et Rhizoma Fagellata, 10 grams of Scutellaria, 6 grams of fried rhubarb, 5 grams of roasted licorice, 50 grams of Chinese yam, 100 grams of powdered kudzu root, 30 grams of stone cassia, 20 grams of Dendrobium Chuan, 7 doses.  On November 3, 2015, after the follow-up, the blood glucose dropped to 6.2 mmol/L fasting after the former medicine, the patient fed back that the whole person felt relaxed and comfortable after the former medicine, headache, eye distension and pain as well as sleep has been normal, still have a little weakness, stool once every two days, tongue coating has not changed, but the pulse has changed from string pulse to slightly slippery, change to use Yu Liquid Tang to benefit Qi, nourish Yin and clear heat to regulate.  Note: Professor Liu Zhilong’s clinical use of sutra prescriptions is often based on his method, not on his prescriptions, and his clinical use of sutra prescriptions is very flexible. In this case, Prof. Liu Zhilong seized that “the patient’s headache is mainly around the solar plexus, and the headache is aggravated by anger and accompanied by eye distension”, which is caused by liver heat rushing upward, and combined with the patient’s poor sleep, weakness, and dry stool once every three days, he identified the patient as having heat in Shaoyang-Yangming syndromes, and chose Chai Hu plus Long Bone Oyster Tang to release heat and subdue rebelliousness, pass stool and calm the mind. The use of stone cassia instead of dragon bone in the original formula, because the stone cassia can not only calm the mind but also clear the liver and eyes; the use of Chinese yam, powdered kudzu root, Chuan dendrobium instead of ginseng, cinnamon branch, poria, jujube, ginger and other drugs that build the middle jiao in the original formula, from the sweet and warm nature to sweet and flat more suitable for the patient’s condition, and the effect of Chinese yam, powdered kudzu root, Chuan dendrobium to reduce sugar is already known to the public.  As for the large amount of Chinese yam and powdered kudzu root, it is related to the culture of Guangdong soup making, people in Guangdong like to use Chinese herbal medicine to make soup, and it is common to use one or two pounds of Chinese yam and powdered kudzu root to make soup, so Chinese yam and powdered kudzu root are dual-use medicine and food, although the amount is large when prescribed, the safety of the medicine is guaranteed, and Professor Liu Zhilong found in the clinic that the effect of lowering sugar is better when the amount is large, so the blood sugar has been reduced to normal level at the time of follow-up. Therefore, the blood glucose was already normalized at the time of follow-up.
