In my orthodontic patients, a large proportion of patients involved in the need to take orthodontic extraction, so patients and parents do not understand, they feel that it is a pity to remove so many healthy and intact teeth, and some parents will ask, after the extraction of such a large gap how to do, but also to set teeth? Some of the old people in the family are even more distressed to hear that so many teeth have to be extracted and they are determined not to let it happen, saying that the teeth will be loose and will fall out early in the future. On this issue, I would like to talk with you who are worried about why it is necessary to pull out teeth for orthodontics. When we go to the doctor for tooth pain and cavities, the doctor usually tries to treat the tooth as much as possible and does not pull out the tooth easily, even if the crown is mostly rotten, if the root of the tooth is fine, it does not need to be pulled out. In contrast, orthodontists often have to extract 4 bicuspids for orthodontic needs, or sometimes they need to extract wisdom teeth that have not yet grown. In fact, tooth extraction orthodontic treatment in orthodontics is very common, about 65%, and even without orthodontic treatment, most people with poorly positioned wisdom teeth have to be extracted. Extraction orthodontics has withstood the test of clinical practice in the history of orthodontic development. Some diseases such as severe crowding and protrusion of teeth must be orthodontically treated by tooth extraction. Without extraction, not only will the deformity not be corrected, but some of the teeth that have been crowded with great difficulty will soon relapse back to their original deformed appearance. Some cases of protrusion, retrusion, and open jaw will not be corrected. After the teeth are extracted there are gaps, which are generally used to move the teeth for orthodontic purposes without veneers, and the gaps will mostly close completely after the orthodontic treatment is completed. Therefore, the gaps provided by tooth extraction are used to (1) correct crowding; (2) correct facial protrusion; (3) correct occlusal relationship: some patients have very flush teeth, but some open jaw, anti-jaw, no occlusal relationship, this situation is also more common to extract teeth. Therefore, the orthodontist decides to extract teeth and non-extractive orthodontic treatment before the individualized orthodontic treatment plan is developed through the patient’s detailed and meticulous examination results, diagnosis and analysis.
