Can you drill and drain a subdural hematoma with aspirin?

Drilling and draining of subdural hematomas is generally not recommended for patients taking aspirin, but may be performed if the condition of an acute subdural hematoma is severe. Since the inhibitory effect of aspirin on platelet aggregation can last for many days, performing the procedure may lead to intraoperative bleeding or increase the risk of postoperative bleeding, therefore, in general, drilling and draining surgery is not recommended for patients taking aspirin combined with non-acute subdural hemorrhage. However, in the case of an acute subdural hematoma with severe injury, rapid progression of the disease, obvious symptoms of increased intracranial pressure, worsening of impaired consciousness, and coma and brain herniation, when the patient’s risk of death is high at this point in time, emergency drilling and drainage surgery should be performed to save the patient’s life, even if the patient is taking aspirin. Whether or not a patient taking aspirin can undergo subdural hematoma drilling and drainage surgery needs to be consulted and clarified with the doctor.
